Narayanan, Namo Narayanan, is the son of State Minister Pandiyanar in Vellore. He has leveraged his father’s name and political reach to amass a vast fortune and an industrial empire through corruption, much to Pandiyanar’s dismay. Although Pandiyanar is an MLA and minister for only three years, Narayanan’s rapid rise casts a long shadow over the region, and the balance of power slowly tilts in his favor.

One daylight incident sets the story in motion: goons act on Narayanan’s orders to finish off a man in public, leaving the city stunned as a witness reports the crime to an ambulance crew. The driver, Maha Vishnu, Samuthirakani, is a compassionate and steady presence who has never let a single patient die under his care. He admits the wounded man to the hospital, a choice that triggers Narayanan’s anger because Vishnu saved his supposed enemy. Vishnu’s response is simple and steadfast: saving lives is his duty, a principle that guides every call he answers.

Vishnu is married to Balambigai, Sunaina, and his home life centers on a loving family, including his father, whom Vela Ramamoorthy portrays as a respected elder, and his sister, Mahishasuramardini, Arthana Binu, a bright but endangered soul. The sister’s life becomes entangled with Narayanan’s circle when Vicky, Vikranth—a drunkard and jobless youth who happens to be Vishnu’s friend—begins to reform with Vishnu’s encouragement. Vicky enrolls in a medical technician course and, through discipline and newfound purpose, earns the trust and affection of Vishnu’s family.

Meanwhile, Mahi’s friend Gowri, Gouri Nair, faces harassment from Narayanan’s brother, Chinna Pandi, a clash that erupts on a bus when Gowri resists his advances and strikes a blow that ignites a brutal retaliation. In a reckless response, Chinna Pandi is killed during the fallout, and Vishnu arrives to admit him to the hospital. His decision to take the outer bypass to avoid traffic enrages Narayanan, who sees Vishnu’s actions as a direct challenge to his authority. The aftermath leaves Narayanan seething and the public emboldened to defend Vishnu, who is left battered but not broken.

The confrontation deepens as Narayanan blames Vishnu for Chinna Pandi’s death and beats him, only to be saved by the crowd’s intervention. Narayanan makes a vow to eliminate Vishnu, setting the stage for a longer struggle that pits a powerful political network against a principled ambulance worker with a growing network of allies.

The personal cost of this feud lands hard on Vishnu when his father loses his hearing and his wife suffers a miscarriage. These blows push Vishnu toward a more urgent sense of justice. Narayanan maintains his grip on power through a sprawling web of bribes and control over local police and officials, including Inspector Uthaman, a figure whose loyalties Narayanan darkens with money and influence. Vishnu finds an ally in Sub-Inspector Rahman, and together they begin piecing together a case that could topple Narayanan’s empire.

Vishnu’s strategy is careful and public-minded: he declines an offer to join the ruling party, choosing instead to move forward on his own terms. He collects concrete evidence—detailed records of Narayanan’s properties and a damning video of the bribery and insults captured by Narayanan’s auditor, a man who once owed Vishnu a life-saving debt and now aids his cause. As the pieces come together, Vishnu realizes that Pandiyanar’s name was used as a front to channel vast sums of money and purchase assets through illegal means.

Armed with documents and testimony, Vishnu dispatches the evidence to the IT department, Vigilance, and other oversight bodies. He also shares the implications with the wives of Uthaman and other government officials, destabilizing Narayanan’s grip as trust in the political machine begins to fracture. The IT department conducts a dramatic raid on Narayanan’s home, confiscating wealth and arresting him, while Pandiyanar is abruptly dismissed from the State cabinet. Pandiyanar’s political power collapses as Narayanan faces the consequences of his crimes.

Desperation drives Narayanan to escape from the hospital as the state contemplates a staged suicide to salvage its public image. He is pursued by the police, manages a temporary escape, but soon meets a devastating setback in the town center. In a final act of accountability and mercy, Vishnu arrives with his ambulance to intervene once more, offering a chance at redress and recourse as Narayanan fights to survive yet again.

Across a sweep of urban and political landscapes, the film traces a battle between corruption and conscience, showing how a selfless act of care—delivered from an unassuming ambulance—can ripple outward to challenge entrenched power and reshape the fate of many lives.
