In a tense opening, Cole and his accomplice Taylor pull off a drive-by that targets the gang’s barroom leader, killing Rodrigo and leaving Taylor mortally wounded while Cole is injured as they speed away. The shooting triggers a wild pursuit by the bikers, who rain fire on the truck and force a chaotic chase toward the parking lot by the amusement park, where emergency calls flood in and fear settles over the scene.
Meanwhile, former marine Jake Carter, now working as a paramedic, teams up with his partner Zoe Williams to rescue a woman named Ana after a crane collapse crushes her car; Ana dies shortly afterward from blood loss. As they depart, a new distress call comes from the parking lot, drawing them back into the escalating danger.
Arriving at the lot, they find Cole and a dead Taylor, and three gang members converge to intercept them. They attempt a desperate escape in Cole’s truck, but gunfire destroys the engine and their chances of a clean getaway slip away. Alonzo, a ruthless rider, orders them to surrender and warns that they’ll kill Zoe if Carter does not yield the one responsible. Sensing Cole is badly hurt, Zoe urges Carter to stabilize him, and Carter heads back toward the truck to retrieve the medical kit left behind. Murphy confronts him en route, and Carter kills Murphy, buying a momentary reprieve.
Zoe creates a diversion to help Cole hide, but she’s knocked out and taken hostage by Deacon. Two floors away, Alonzo holds Zoe at gunpoint, threatening to kill her if Carter doesn’t surrender the man responsible. Carter uses [Taylor]’s corpse as a decoy and presents him to Alonzo and Deacon, who reluctantly release Zoe. After the immediate danger subsides, Zoe calls Carter to untie her hands, yet Alonzo and Deacon anticipate that they can finish off both paramedics and strike at any moment.
Vincent, the gang’s leader, arrives and Alonzo confirms the job is done, but Vincent reveals there were two attackers. The hunt for the second man ramps up; Carter heads back toward Cole but is intercepted by Deacon. In the ensuing struggle, Cole injects Deacon with morphine and Carter finishes him with an axe, then they retreat into an elevator to regroup.
Carter presses Cole for the reason behind the bikers’ pursuit. Cole admits he killed Rodrigo to protect his five-year-old daughter, explaining that debt and threats from the bikers were the motive, and that the killer was one of their own. After Carter eliminates another biker, they manage to flee the parking lot and push toward the park for cover, where a fierce shootout erupts and Carter’s fate seems bleak, apparently killed.
Inside the escalating confrontation, Alonzo forces Cole into a brutal confrontation; Cole reveals that Vincent had coerced him into the killing. Vincent argues Rodrigo was weak and had allied with other gangs, but Alonzo’s brutality leads to a fatal betrayal: Vincent’s brother Cash turns on him and stabs him to death, declaring it in the name of the Lost Legion, while Cash himself still eyes Cole as a target. The tension peaks as Carter, wounded, crawls toward safety and Cole fights to stay alive.
Recovering enough to act, Carter rescues Cole inside an ambulance and the two make a daring stand to wipe out the remaining bikers, including Cash. They retreat to a construction site to seek medical help, only to be found by Alonzo, who wounds Carter in a brutal fistfight. Cole makes a break for the rooftop, where Alonzo confronts him, but Carter arrives just in time and hurls Alonzo to his death.
As medics arrive and tend to Cole, Carter tells him that he’s going to have to face the police and likely return to prison, but Cole’s gratitude for being saved is clear. He speaks of a chance to watch his daughter grow up, and the two men share a quiet, hard-won understanding that some debts are paid in blood and survival, not clemency.
