Several years prior, Peter Koslow found himself released early from prison, bound by the stipulation that he operate as an undercover informant for the FBI’s New York City field office. As he navigates his way into a Polish crime syndicate, he stands poised to deliver significant evidence to his handler, Special Agent Wilcox, that could lead to the arrest of the crime boss, Ryszard “The General” Klimek. While smuggling a considerable amount of fentanyl through the Polish consulate’s office—equipped with a wire—his plans take a dangerous turn when his associate Staszek diverts from their agreed strategy, pursuing a potential buyer instead.

Amidst this unfolding chaos, Koslow quickly realizes that the buyer is an undercover police officer. Tensions escalate, and in a shocking turn of events, Staszek takes drastic action, killing the officer. Listening in on this grim incident, Koslow’s handlers shockingly decide to abandon him, leaving him unprotected and vulnerable. Subsequently, he and Staszek are apprehended by The General, who imposes a dire choice upon Koslow, demanding that he return to prison to oversee the drug operation on The General’s behalf. Faced with the threat to his beloved wife Sofia and their daughter, Koslow reluctantly agrees, knowing that their safety is at stake.

In a precarious twist, Wilcox informs Koslow that he will continue his role as an informant even while incarcerated, promising his release once the operation has concluded. As the plot thickens, NYPD Detective Grens begins unraveling the murder of the undercover officer, with a growing suspicion that the Polish mafia is behind it. Grens manages to obtain security footage of Koslow leaving the scene, launching his investigation further into Koslow’s entangled life. In the meantime, The General and his wife visit Koslow and Sofia in a seemingly innocent gesture as they prepare for his return to prison.

As the narrative progresses, Koslow makes a bold move, breaking his parole. Grens, crossing paths with Wilcox and Montgomery, faces a dead end as they deny any involvement in the recent murder. Sofia, protective of Koslow, remains tight-lipped when Grens seeks her cooperation. Meanwhile, Koslow’s attempts to establish control over the drug trade within prison are thwarted by a corrupt officer named Slewitt—an enforcer for the prison gang led by Smiley Phelps, who maintains a grip over the local drug distribution network. In a harrowing moment, after nearly being killed, Koslow reveals his informant status to Smiley, positioning himself to eliminate The General, the biggest threat in the operation.

In a further twist, Koslow reaches out to Wilcox for assistance, only to find her bound by orders from Montgomery to distance the FBI from the botched operation, leaving Koslow hanging. Desperate, he contacts Sofia, urging her to hand over the secret audio tapes he recorded during his discussions with Wilcox to Grens. Sensing the looming danger, Wilcox ultimately informs The General’s lawyer about Koslow’s true identity, triggering a death warrant from The General against him and his family.

As the tension escalates, Koslow battles a Polish gangster, managing to subdue him in a fierce struggle. Seeking a way out, he takes Slewitt hostage, leading to a tense standoff. Meanwhile, Wilcox confronts Sofia, taking charge of the situation and seizing the tapes, which may prove vital to their survival. As chaos reigns in the prison, Montgomery steps into command of the hostage circumstances. Utilizing his former training as a US Special Forces sniper, Koslow devises a cunning escape plan, switching attire with Slewitt. This sets off a chain reaction, resulting in a sniper incorrectly shooting Slewitt, triggering an explosive catastrophe that creates a diversion for Koslow’s escape.

In a gripping climax, Staszek and another gangster target Koslow’s family but are intercepted by Grens, who sustains injuries in the process. Fortunately, Sofia manages to assist Grens, and both gangsters meet their demise. Transported via ambulance and groggy from the ordeal, Koslow discovers that Wilcox is now his escort. In a turn of events, she allows him to escape, hoping to atone for the FBI’s previous actions. Upon returning to New York City, Wilcox takes a stand against Montgomery, recording him as he threatens Koslow, ultimately leading to his arrest. As the dust settles, Grens and Wilcox collaborate to assist Koslow in safeguarding his family, preparing to weave him into hiding until the FBI completes its investigation into the corruption that nearly cost him everything.
