Tom Ludlow, portrayed by Keanu Reeves, is an L.A. Police Officer caught in the grips of disillusionment and haunted memories of his late wife. The film begins with Tom waking up after a night of heavy drinking, showing his reckless nature. In a tense undercover operation, he collides with two Korean gangsters in a parking lot, who are keen on purchasing a machine gun from him. However, the situation spirals out of control when the gangsters brutally assault him and make off with his vehicle.

Determined to retrieve his stolen car, Tom tracks it down to the gangsters’ hideout, a daring move that leads to a violent confrontation where he kills everyone in the house, including an unarmed individual caught off-guard in the restroom. Quickly, he locates two missing children imprisoned there, but he opts to mask the truth of his actions with a pair of rubber gloves, highlighting his moral conflicts.

In the aftermath, Tom finds himself amidst a police investigation at the crime scene, where his former partner, Terrence Washington, played by Chris Evans, grows concerned about the rampant corruption within their unit led by Captain Jack Wander, played by Forest Whitaker. Washington’s newfound moral compass leads him away from the corrupt practices that dominate their work environment, drawing Tom’s ire when he learns that Washington has reported him. While Wander congratulates Tom, he brings him to the hospital for treatment of a wound sustained in the gang fight, where he encounters his girlfriend, Grace Garcia.

Things take a turn at a department party when Tom learns that Internal Affairs Captain James Biggs, portrayed by Hugh Laurie, is investigating him due to Washington’s testimony. Out of anger towards Washington for his apparent betrayal, Tom confronts him, but their skirmish is interrupted by a robbery. Tragically, in a chaotic turn of events, Washington is murdered, and Tom is implicated when Wander encourages him to steal security footage that could incriminate him.

As the investigation deepens, Tom is determined to prove his innocence. He enlists the help of Detective Diskant to chase leads, which culminates in a shocking discovery of two bodies in the hills—those of the true criminals, Fremont and Coates, who were killed before Washington’s murder. As Ludlow and Diskant maneuver through the dangerous web of deceit, the situation takes a tragic turn when Disco is also killed in a firefight with the imposters posing as the criminals.

With mounting pressure and a narrative painted against him, Tom returns to Grace’s house, only to be ambushed by corrupt officers from his unit. He soon realizes that the web of corruption runs far deeper, including his own colleagues attempting to erase him. Managing a narrow escape, Tom finds himself embroiled in life-and-death confrontations as he battles those who’ve betrayed him, ultimately leading him to Captain Wander’s house, where the true extent of the corruption is finally unveiled.

In a gripping conclusion, Ludlow is forced to confront the betrayal of his friend Wander, who reveals a cache of stolen money and other incriminating evidence against the entire department. The revelation that he was mere collateral in a larger scheme unnerves Tom, culminating in a desperate showdown leading to Wander’s demise. Finally, as Internal Affairs closes in, neither side walks away entirely unscathed, but one thing remains clear: the complexities of loyalty, betrayal, and justice are tangled in the bleak world of corrupt policework. In the end, Internal Affairs uses Ludlow to take down Wander, allowing him to understand the origins of corruption within his own ranks.
