Al Simmons, portrayed by Michael Jai White, is a military soldier and assassin who finds himself the victim of betrayal by Jason Wynn, the head of a covert government agency, played by Martin Sheen. During a dangerous mission to eliminate a Bio-Chem plant in North Korea, Wynn conspires with his top assassin, Jessica Priest (Melinda Clarke), to have Simmons killed. Following a destructive gas fire orchestrated by Wynn, Simmons meets his end. He then arrives in Hell, where the Devil himself, Malebolgia (Frank Welker), presents him with a tempting offer: serve as the leader of his army during Armageddon, and he can return to Earth to see his beloved fiancée, Wanda Blake (Theresa Randle). Simmons, driven by longing, accepts the pact and is resurrected.
Upon his return, Simmons discovers that five long years have elapsed, during which Wanda has moved on and married his best friend, Terry (D. B. Sweeney), creating a family he never knew he desired, complete with a daughter named Cyan (Sydni Beaudoin). In this new reality, he meets a bizarre clown-like demon known as The Violator, brought to life by John Leguizamo, who begins to lead him down a dark path. Along his journey, he encounters Cogliostro (Nicol Williamson), a fellow Hellspawn who aids him and fights for Heaven.
Meanwhile, Jason Wynn has reinvented himself as a prominent arms dealer and has developed a catastrophic biological weapon named “Heat 16”. At a lavish reception, a furious Simmons confronts Wynn, kills Jessica, and escapes, unexpectedly harnessing the power of Spawn’s mystical armor. However, The Violator convinces Wynn to implement a device that will unleash Heat 16 globally if Wynn’s heart stops, a scheme designed to draw Simmons into a lethal confrontation.
Tensions escalate when The Violator battles Spawn, only to be saved by Cogliostro, who instructs him in the use of his necroplasm armor. In a desperate turn of events, Clown and Wynn plot to eliminate Terry. As Terry receives a crucial text exposing Wynn’s corrupt actions, he becomes entangled in a deadly trap when Jason arrives, seizing control of the situation. When Spawn intervenes, he nearly executes Jason but recoils, realizing that doing so would endanger Cyan’s life. Instead, he extracts the lethal device from Wynn before obliterating it.
In a stunning series of encounters, Spawn confronts Malebolgia, declaring he will not succumb to his desires for power.
