Nuar and Nur are twins. Nuar, a seasoned cop, grows weary of Nur, a drug addict who continually slips away from rehab to hide in a warehouse. After a heated argument, he sends Nur back to the rehab center, hoping for a safer path for her. The police dispatch crackles to life with reports of a foul stench—a sign that something terrible has happened—and Nuar follows the calls to an abandoned house. Inside, he confronts a string of eerie sights and sounds, including the unsettling noise of a marble rolling and dropping on a floor, but he tells himself not to overreact. On the second level, he discovers a girl, Balqis, chained and clinging to a corpse, while her deceased twin sister Qistina lies nearby. The revelation shocks him: Balqis and Qistina are the daughters of Megat Mahmud. This revelation deepens the mystery, as Nuar realizes that no one had ever reported the twins missing.

As night settles, the quiet around home grows strange. Nuar returns to his routine, but the world seems unsettled by unseen forces—echoes, whispers, unexplainable cold spots—and a ghostly presence grips his hand, leaving a blue mark that sears its memory into his skin. The following evening, Balqis is possessed and vomits blood, while a note surfaces indicating Faizah’s spirit is targeting Khaleeda. Alarm bells ring in the police station as Nuar, accompanied by Luqman and another officer, rush to Faizah’s house to intervene. In a tense confrontation, Faizah’s spirit lashes out, but Luqman steps in with prayers to exorcise the spirit, leading to Faizah’s arrest.

The investigation continues the next day, and Nuar sits down with Faizah for a police interview. After Faizah appears to be possessed again, she attacks him and tries to flee, eventually dying in a car accident. Balqis returns to Nuar with a crucial, tragic detail: their parents were killed in a road accident years earlier. Opah then locks Balqis and Qistina inside the abandoned house for safety, a decision that weighs heavily on the case as Faizah arrives later with food and is shocked to discover Qistina’s skulls, a grim reminder of the twins’ fates.

Suspicion falls on Nuar, and he is arrested by ASP Mansor for the murders of Opah and Megat Mahmud, shattering his sense of justice. At the same time, inside the same haunted place, Nuar and Lokman discover that Khaleeda has been brought to the house by Balqis. Lokman is soon attacked by the Balqis spirit, pushing the investigation toward a deeper, more dangerous truth. In the midst of these supernatural and human dangers, Nuar finally comes face-to-face with Nur, who appears alongside Balqis’s spirit in a final confrontation that tests his will to survive. As the conflict escalates, Nuar shoots Balqis, and Nur’s spirit launches a renewed attack on him. A cabinet collapses onto Balqis, allowing Nuar to seize Balqis’s ring just as he races to rescue Nur. Luqman arrives and saves Khaleeda, pivoting the danger toward a fragile relief.

Ultimately, Nur is cured, but Nuar sustains injuries. In the two months that follow, Nuar recovers enough to return to the police, while Nur nurses a broken arm. The two siblings share a quiet moment on a bench with Luqman, trying to make sense of the ordeal. In the mid-credits scene, Khaleeda boards a bus, and a chilling reminder lingers: Balqis has returned.
