In March 2007, during a stay in San Diego—the setting of the first film—Haruka Yamano Noriko Aoyama is involved in a car accident that leaves her with two broken legs. She returns to Tokyo in the care of her 19-year-old brother Koichi Yamano Aoi Nakamura after their father Shigeyuki Yamano Kazuyoshi Tsumura leaves on a business trip abroad.

One morning, a troubling change appears in Haruka’s life: her wheelchair slides across the floor even though its wheels are locked. Koichi suspects something paranormal and places a mound of salt in her room as a precaution, only to find it scattered the next day. Haruka dismisses the idea, but Koichi’s curiosity grows, and he continues filming. Their camera captures eerie moments—the glass cup shatters during dinner, and soon, a pattern emerges: several objects around the house seem to move on their own whenever the footage is reviewed.

The tension escalates when Koichi’s friend Jun Nagoshi Kosuke Kujirai visits with his girlfriend and Misuzu Kure Ayako Yoshitani. Misuzu experiences a terrifying episode, screaming and foaming at the mouth as she investigates Haruka’s room. In response, Koichi seeks help from a Shinto priest to perform a purification ceremony, which provides a temporary calm. After the ceremony, the siblings breath a bit easier as two peaceful days pass, but their relief is short lived—their father Shigeyuki returns home, and the strange occurrences begin to intensify once more.

On night 12, Haruka’s wheelchair slides toward the camera and cuts the footage, signaling that the presence has not left the house. The priest later dies of a heart attack after leaving them, leaving the siblings to face the haunting without its ritual protection. The incidents grow more violent: Haruka is dragged out of bed by her hair, and Koichi wonders if the demon tied to the family’s tragedy has followed them from the accident in America.

As Haruka’s condition seems to worsen, she recalls Katie from the first movie—a woman who suffered a similar fate in the car crash that disrupted her own life. Through online research, Haruka believes the same demon may be targeting them. She reveals a hospital memory of a bite mark on her arm, and Koichi places a crucifix in her hand. When he leaves, Haruka drops the crucifix, which instantly bursts into flame and causes shattered windows, a shocking sign that deepens Koichi’s fear and resolve.

Around night 15, Haruka awakens and lingers by Koichi’s bedside for hours before he heads downstairs. There, he discovers their father’s corpse hidden in a closet. The possession intensifies: a furious Haruka becomes violent, forcing Koichi to flee and hail a taxi for safety. As he escapes, Haruka suddenly appears in front of the vehicle, and the taxi strikes her, sending the car into a crash.

Koichi later returns to a funeral home to pay his respects, thinking Haruka has died in the crash. When he lifts the sheet, he discovers the body is not Haruka but the taxi driver.
