Freelance investigative journalist Miles Upshur receives an anonymous tip about inhumane experiments at Mount Massive Asylum, a remote psychiatric hospital run by the unscrupulous Murkoff Corporation. Upon entering, he finds the halls in disarray and the staff’s bodies mutilated, a horror scene that hints at something far beyond ordinary care. A dying officer warns that Murkoff’s private military unit has escaped into the grounds, and that the main entrance can be unlocked only from the hospital’s security control room.

Miles presses on, only to be ambushed by the hulking variant Chris Walker, who knocks him unconscious. While unconscious, Miles meets Father Martin Archimbaud, a self-appointed priest with schizotypal traits who declares Miles his apostle and attempts to derail his escape by cutting power to the entrance. When Miles restores power in the control room, Father Martin injects him with an anesthetic and shows footage of the Walrider, a ghost-like entity that allegedly slays patients and staff alike, insisting that this being is responsible for the building’s ruin.

When Miles awakens, he is trapped in a decaying cell block filled with catatonic and demented patients. He fights his way through the sewers to the main wards, pursued by Walker and two cannibalistic twins, only to be captured by Richard Trager, a former Murkoff executive driven mad by the experiments. Trager amputates two of Miles’ fingers and threatens further mutilation. Miles escapes to an elevator, and in the chaos, he unintentionally crushes Trager to death between the floors.

Miles reunites with Father Martin, who directs him toward the hospital chapel for safety. In a crowded auditorium, Miles learns that the Walrider was created by Rudolf Wernicke, a German scientist brought to the United States during Operation Paperclip. Wernicke explains that an intensive form of dream therapy conducted on traumatized patients could fuse swarms of nanites into a single malevolent being, a project rooted in the darker chapters of MKUltra. In the chapel, Miles discovers a crucified Father Martin, who hands him a key to the lobby elevator and then immolates himself before Miles takes the ascent to a subterranean laboratory.

The elevator ride brings a confrontation with Walker, but the Walrider claims the predator’s life in a brutal strike. Miles then meets an aged Wernicke, who confirms that the Walrider is a biotechnological nanite entity controlled by Billy Hope, a comatose patient of Murkoff’s experiments. Wernicke urges Miles to terminate Billy’s life support in the hope of destroying the Walrider. Miles carries out the order, yet as Billy dies, the Walrider ambushes him and possesses Miles’ body. He ascends from the laboratory only to be gunned down by a Murkoff private military unit led by Wernicke.

The screen cuts to black as panicked screams, gunfire, and the sounds of a savage mauling fill the air. The Walrider, a biotechnological nanite entity, has found a new host in Miles, and the story leaves his fate—and the hospital’s dark experiments—shrouded in unresolved dread.
