Zach Riley, Aaron Eckhart, is a psychiatrist who leaves a prestigious university to take a position at Millwood, a privately run mental institution overseen by William Hurt as Dr. Peter Reed. What he doesn’t reveal at the outset is that Millwood is the very place where his novelist father, Nick Nolte as T. L. Pierson, spent years battling depression. Pierson later wrote a popular children’s classic, Neverwas, about a child (based on Zach himself) who enters a hidden world to free a captive king. Riley arrives with a sense of personal history weighing on every decision, and the new job becomes a way to confront the past while tending to patients.

Riley is soon paired with a schizophrenic patient, Gabriel Finch, who sees himself as the captive king; Ian McKellen brings this character to life. As he continues his work, he rekindles a long-ignored connection with his childhood friend Maggie Paige, Brittany Murphy, and begins to notice that Finch’s memories are more deeply tied to his own history than he realized. Finch’s stories, Riley learns, helped form the book his father wrote—stories Finch told while both were patients at Millwood—creating a bridge between fiction and the hospital’s reality.

Finch treats the book as an oracle that confirms his own reality, and he is convinced that Riley is the boy hero at the center of the tale. As this belief deepens, Riley starts to understand that Finch’s hallucinations may point to real locations and events rather than fantasies, forcing him to reassess his own life and the bonds to his father’s work. The mystery thickens as what began as professional care begins to resemble a revelation about memory, healing, and the way stories shape the people who tell them.
