Victor, Ishan Shoukath, a blind man, witnesses the murder of his close friend Wasim. Despite his blindness, Victor identifies the killer by the distinct scent of the perpetrator’s perfume and by recalling the killer’s vehicle, a Land Rover Defender. This clue pulls the police toward Russell, Abhimanyu Thilakan, who had conspired with Wasim’s elder brother Tariq Arjun Nandhakumar to carry out the crime. Tragically, Russell captures Victor and dumps him into a tub of hydrofluoric acid, ending Victor’s life and sending shockwaves through Kochi’s crime underworld.
George D’Peter, Siddique, the patriarch of the Adattu family, is left utterly shattered by Victor’s death. Marco Jr., Unni Mukundan, Peter’s adopted brother who had been sent away to Italy for safety, returns to India after learning what happened. At Victor’s memorial service at their local church, Marco vows to avenge Victor’s death and to take down everyone involved, starting with Russell.
Tony Issac, Jagadish, Russell’s father and the head of the Isaac gold-smuggling syndicate, plays a dangerous game of manipulation. He orchestrates a deal with Peter, exchanging gold biscuits and a ransom while secretly plotting further treachery. On Peter’s return from the deal, Devraj, Anson Paul, an Isaac henchman, ambushes him and reveals the conspiracy, leaving Peter wounded and hospitalized and fueling Marco’s desire for revenge. Peter makes Marco understand that Tony and Russell were responsible for Victor’s death.
Marco sets out for vengeance, and the feud intensifies as Russell strikes back, maiming Marco and injuring Anwar, Marco’s right-hand man. Marco retaliates by cutting through Russell’s men with brutal efficiency, using the same ruthless force that has defined this war. He learns that Isha, Victor’s pregnant wife, is being held captive by Tony, who demands the Adattu family’s entire gold empire in his name in exchange for her release.
Marco captures Devraj and tortures him with phosgene gas. Devraj finally coughs up that Isha is at Tony’s guesthouse, and Marco storms the guesthouse to rescue her, slaughtering Tony’s henchmen along the way. Yet Tony proves cunning, deceiving Marco by claiming Isha is actually locked in the trunk of Russell’s car. Undeterred, Marco and Anwar pursue Tony to a farmhouse, where Tony reveals a shocking truth about his sexuality and attempts to seduce them; Marco responds by severing Tony’s right hand with a katana and sending it to Russell, disguised as a bundle of company documents.
In a final trap, Marco rigs Tony’s chair with a detonator. When Tony is hoisted into the air, the detonator explodes, leaving Tony mortally wounded as Russell looks on helplessly from outside the warehouse. Around this time, Peter exacts justice on Tariq by hanging him.
Desperation and grief push Russell to seek aid from Cyrus, Kabir Duhan Singh, a ruthless and psychotic butcher. They storm the Adattu mansion in the throes of Isha’s labor, slaughtering the entire family, including the children and Maria, and abducting Isha’s newborn child. Marco and Peter, though gravely injured, muster their strength to defend what remains of their clan, but Cyrus and Russell overwhelm them and leave with the baby.
With the Adattu family nearly wiped out, Marco and Peter lie battered but unbroken, and Peter urges Marco to avenge not only Victor but the entire family. Marco emerges transformed, adopting a terrifying disguise as he confronts Cyrus and Russell at their factory. In a bloody, no-holds-barred showdown, Marco defeats Russell, ripping out his rib cage with a crowbar and tearing out his heart with his bare hands. In a climactic duel with Cyrus, who holds the newborn over a pool of acid, Marco decapitates Cyrus and saves the child, paying the ultimate price with the heavy losses suffered by his own family.
In a post-credits moment, Marco spends time with Victor’s child on a quiet beach, a somber pause before a platoon of cars rolls in, signaling that the cycle of vengeance may be far from over.
