Lyon Gaultier, Jean-Claude Van Damme, a hardened member of the French Foreign Legion stationed in the arid reaches of Djibouti, receives devastating news: his brother in Los Angeles is dying after a drug deal goes wrong, and a voice seems to call his name in the chaos. A fearless breakout from the base propels him into a perilous desert trek toward the coast, where a battered tramp steamer waits to ferry him toward the United States. Anticipating his escape, Lyon’s Legion Commander dispatches two of his own men to pursue and bring him back for a court-martial, setting a clock on his fragile freedom.
Arriving in New York City with nothing but grit, he is drawn into an illegal street fighting scene run by a calculating promoter. He volunteers for the next bout and overwhelms his opponent, catching the eye of Joshua Eldridge, a rugged tramp who runs the ring. Joshua introduces him to Cynthia Caldera, an unscrupulous organizer who sponsors Lyon, dubbing him “Lionheart” and pitting him in a no-holds-barred brawl against Sonny. The spectacle is brutal, and Lyon’s display of power signals the start of a dangerous ascent through the shadowy world of elite underground fighting.
After the victory, Lyon and Joshua push north in search of a phone booth to contact Hélène, fending off a local street gang along the way. Joshua calls in a favor from Cynthia, who helps them cross the country toward Los Angeles. By the time Lyon reaches the hospital, his brother has died; the murderers are in custody, but Hélène is left penniless with a young daughter, Nicole. Lyon tracks down Hélène’s address, and when he speaks to her, she lashes out, accusing him of deserting his brother and blaming him for the troubling life surrounding her family.
Determined to help without drawing attention, Lyon uses Cynthia’s network to enter the local street-fighting circuit and discreetly delivers his winnings to Hélène in the form of checks, with Joshua suggesting their late husband subscribed to a life insurance policy. Lyon defeats a string of high-profile fighters—a dirty-fighting Scotsman, a wrestler in a squash court, and a martial artist in a shallow pool—while the money quietly finds its way to his sister-in-law and niece. Cynthia grows increasingly suspicious of Lyon’s generosity toward Hélène and becomes jealous of the bond he forms with Nicole, sending her assistant Russell to tail him, while two Legionnaires stake out Hélène’s apartment and threaten to seize Lyon.
Cynthia arranges for Lyon to face Attila, an undefeated combatant whose style toys with opponents by giving them a false sense of opportunity before finishing them off with ruthless precision. The promoter shows bettors an altered tape to cast Attila as the weaker fighter, and she bets her entire fortune on him. Realizing Lyon is wounded, Joshua tries to talk him out of the fight, but Lyon pushes through, summoning the last reserves of strength.

The Legionnaires close in, but, moved by his niece’s tearful farewell, they release Lyon a few blocks away and wish him luck in his new life in America.
