Sam Butler, Ari Levin, returns to his Las Vegas home after a successful business deal in Sacramento and stumbles upon his pregnant wife, Ruth, in bed with a sheriff’s deputy. Whipped into a homicidal rage, he seizes the deputy’s discarded gun and fires, killing both the deputy and Ruth. He then beheads Ruth, seizes the severed head, and drives off with it.

At a motel, he picks up a prostitute named Tina. Back at the motel’s rear, Sam ties Tina up and stabs her in the jaw, then rips one of her legs off by attaching it to the back of his car. The spree continues as he shoots a bartender in the head and then traces a car advertising the Beautiful Lady Oil Wrestlers to an apartment complex. There, a gardener’s head is crushed with a shovel, marking the start of a brutal confrontation.

Inside the apartment, the oil wrestlers are hosting a baby shower for Barbara, the first to fall when Sam breaks in. After a failed attempt to rape her, Barbara’s unborn child is somehow cut out, and Sam proceeds to murder the rest of the group with whatever tools he can find. He uses a knife, a drill, a hammer, a gun, and his bare hands in rapid succession, taking down five wrestlers and a man who comes to their aid, culminating in the beheading of a visiting Jehovah’s Witness.

The body count climbs as Sam roams from room to room, leaving a trail of blood and wreckage behind him. The chaos is punctuated by the bleak, chilling sounds of the night and the harsh reality of his violence. The movie lingers on the aftermath, with Sam continuing to commit carnage as police and bystanders struggle to respond to the escalating horror.
