Mohanlal as Madhava Menon, also known as Mad Maddy, is a ruthless and psychotic cop with the Mumbai Police, an encounter specialist who makes on-the-spot decisions rather than waiting for orders from above.

His current case centers on the disappearance of a teenage Mumbai schoolgirl, and a tip steers him toward Kerala, where authorities believe the girl could have been taken. The mystery deepens as another girl is kidnapped in Kerala, suggesting a broader pattern and hinting that multiple victims may be connected to the same network.

Following the leads, Maddy concludes that a Kerala-based sex trafficking racket is behind the disappearances, led by a brutal pimp named Khais Khanna, who bears a personal grudge against him. In the past, Khanna abducted a girl, and Maddy intervened to rescue her. When Khanna tries to strike again, Maddy confronts him in a brutal clash that ends with Maddy stabbing Khanna in the groin, leaving him with a permanent injury.

Driven by duty and a sense of justice, Maddy dedicates himself to dismantling the racket and saving the girls who have fallen victim to it, including his own daughter. The danger hits close to home as Diya becomes entangled in the case, fueling his resolve to push forward.

With the support of his fellow officers, Maddy closes in on the criminals, exposes the network, and ultimately brings the gang to justice. He defeats Khanna and rescues his daughter along with the other abducted girls, ensuring they are freed from exploitation and given a chance at a safer future.
