Jessica Angie Dickinson is a nurse who, after her husband dies in Sicily on their honeymoon, chooses to stay rather than return home and settles in the village of Forza d’Agrò to work as a midwife.

Her striking presence draws the attention of the townsmen, which breeds resentment among the women. While she sometimes welcomes the interest, she maintains a strict rule: she will not become involved with any man who has any attachment to another woman. The wives hatch a plan to abstain from relations with their husbands, arguing that if no babies are born, no midwife will be needed. The parish priest, Father Antonio Maurice Chevalier, disapproves of their scheme.

Jessica develops a romantic fascination with Edmondo Raumo [Gabriele Ferzetti], a marchese who has lived as a recluse since a war injury. Raumo hides his true identity, telling Jessica that he is a humble fisherman. When the truth finally comes out, Jessica is angered and contemplates leaving Sicily for good. Yet a dying villager who has trusted and helped her pulls her back toward Raumo, softening her resolve and deepening their bond.
