In 1990, on the island of Mauritius, a woman named Susan Alves is confined to a mental asylum, frantically scrawling demonic images on the walls of her cell. Her husband, Joe Alves, comes to visit and the couple speaks about their daughter, Pia. Overwhelmed by fear and despair, Susan finally collapses into a panic and, in a shocking moment, takes her own life in front of Joe.

Jumping to present-day Mumbai, Pia Mugdha Godse is married to Vic Bobby Deol, a horror film director, but the couple’s relationship is strained. When Pia’s friend Carol Sophia Handa from Mauritius reveals that Pia’s father Joe has suffered a heart attack, they travel to the island and return to Pia’s childhood home. It’s there they uncover a painful fragment of Pia’s past: she once had a twin sister named Dia, with whom she was inseparable. That night, Pia experiences a nightmare of a girl’s ghost, and inanimate objects in the house begin to move on their own. The next morning brings a disturbing flashback: Joe carries a dead Dia out of a pool, a memory that haunts Pia because she recalls being with Dia at all times—except the day Dia drowned when Pia was not by her side.

During dinner, Pia steps outside to check on Tracy, the family dog, which is barking at something unseen. She encounters the girl’s ghost again, a sign that something supernatural lingers in the house. Pia soon learns she is pregnant, a revelation that deepens the emotional strain between Pia and Vic. Joe shares old photographs with Vic and points out Dia’s favorite doll, which had been buried with Dia. Pia breaks the news of her pregnancy to Vic, and the joy temporarily mends their strained relationship, renewing their sense of happiness.

Yet the house continues to be unsettled by inexplicable events. One night, Dia’s ghost drops down on Pia from the ceiling, a chilling image that unsettles her. The next morning, Pia seems markedly changed. When Tracy is found dead, Pia’s reaction appears oddly calm, unsettling Carol enough to seek help. Carol enlists her friend Aditya, a parapsychologist, for assistance, hoping he can decipher the sinister happenings orbiting Pia.

Carol’s encounter with the ghost forces her to flee, but tragedy strikes when she is hit by a truck and dies. At the funeral, Aditya senses that Pia’s life is in danger and urges Vic to seek his help. After encountering the ghost himself, Vic turns to Aditya for guidance. The two dig through the attic and uncover a troubling trove: the doll buried with Dia, a videotape, Susan’s diaries, and disturbingly morbid paintings by Susan depicting suicide and inner turmoil. Vic visits the nearby mental asylum, where the janitor explains that Susan was not insane but possessed by an evil spirit.

Vic and Aditya proceed to watch the video tape, which appears to show Pia drowning Dia and then flashing an eerie, self-satisfied smile. Yet they sense the tape is only telling part of the truth. Repeated viewings reveal a deeper pattern: Susan was already possessed before Dia’s death. Aditya notices a reflection of a third girl in the mirror—an image that cannot be Pia, who is filming, and cannot be Dia, who stands before the camera. The implication is clear: it is a malevolent spirit, meaning Pia was not the killer, and that Dia’s message was really about a hidden murderer.

Vic learns that the women in Susan’s family become possessed during pregnancy. A possessed Pia ends up killing Joe. The truth finally emerges: Susan had been pregnant with triplets. Dia and Pia were born without issue, but the third child suffered grave complications, and Joe had to choose to save Susan over the unborn child. Realizing that the triplet’s vengeful spirit is at work, Vic and Aditya fight to banish it. Aditya is killed in the struggle, but Vic manages to trap the spirit in the other realm.

Seven months later, Pia gives birth to a baby girl. Yet, as Pia sleeps, the newborn thrashes, and its eyes go blank in a way that mirrors the spirit’s presence—suggesting that the malevolent force may have taken up residence in the child. The haunting conclusion leaves a foreboding sense that the cycle of possession and revenge will continue, threading the past and present together in a chilling tether.
