It all kicks off with a TV news anchor recounting the harrowing tale of Flight 180 and its unfortunate survivors, culminating in the lone survivor, Clear Rivers. Following this, we meet Kimberly, portrayed by A.J. Cook, as she embarks on a road trip with her friends. During their journey, while driving with one friend and two stoners in the back, a call from her father alerts her to low transmission fluid, which she acknowledges but ignores as her friend advises against stopping the car.

As Kimberly continues driving, she encounters a variety of characters, including a bus of exuberant football players, a jock, a cyclist, a mother with her son, a truck loaded with logs, a drinker in a truck, a stoner, and a cop. The disaster strikes when the cop spills his coffee, causing the cargo from the truck to break loose. In an instant, chaos ensues—logs crash through windows, cars flip, and the unthinkable happens as her friends are caught in the mayhem. Meanwhile, Evan, the fortunate lottery winner, is facing his own challenges as strange incidents plague his luck—his microwave explodes and he gets his hand stuck in the garbage disposal. Narrowly escaping disastrous luck, he climbs down a fire escape only to have the ladder nearly kill him.

As we shift to Thomas, he investigates the harrowing history of Flight 180, uncovering that Alex Browning, a survivor, met his end in an unfortunate incident involving a falling brick. Meanwhile, the other survivors of the car crash, including Kimberly, watch new reports, and Kim, obsessed with the concept of survival, takes a trip to see Clear Rivers at the mental institution. Clear cautions Kimberly, insisting that only she can save herself, implying that her friends are doomed.

The plot thickens as dreadful events unfold—Tim suffers near-death experiences at the dentist, while Nora experiences a hair-raising incident involving a bird and an elevator. In a rush of fear and urgency, they all gather with Clear to confront their fears and potential fate, encountering William Bludworth, who cryptically states that to elude death, new life must be created.
