Ultimately, they encounter a compassionate surgeon who is touched by Manitas’ heartfelt stories of her childhood experiences with gender Diaspora, leading to a crucial moment where the doctor agrees to perform the transformation.

With the surgery completed, Manitas steps into her new identity as Emilia Pérez. In an effort to shield her family amidst the dangers of her past, Emilia’s wife, Jessi, and their children are relocated to Switzerland, while Rita is significantly compensated for her efforts as Emilia orchestrates her own disappearance to sever ties with her former life.

Fast forward four years to London, where fate reunites Rita with Emilia in a twist of circumstances. Emilia confides in Rita her longing to reconnect with her children. Touched by her sentiment, Rita makes arrangements for Jessi and the children to return to Mexico City, presenting Emilia merely as a distant cousin of Manitas.
