Dr. Caligari Madeleine Reynal presides over the C.I.A. (Caligari Insane Asylum) with a controversial method that pushes the boundaries of psychiatry: transferring glandular brain fluids from one patient to another in a mind-bending attempt to swap core traits and reform disturbed minds.

Two central patients, Mr. Pratt John Durbin and Mrs. Van Houten Laura Albert, become the primary subjects of the mindswapping experiment. After the procedure, Mrs. Van Houten shifts into a cannibalistic harshness, while Mr. Pratt swings toward an intensified nymphomaniac impulse; both menacingly carry echoes of who they once were, suggesting that identity isn’t completely erased by the swap but rather redirected. The film hints at a larger theory behind Caligari’s method—an attempt to cure by balancing opposite traits—but never lays out a clear, explicit explanation.

As concerns grow, the married couple Ramona Lodger Jennifer Balgobin and Dr. Lodger [David Parry] become increasingly uneasy. They seek accountability and turn to Ramona’s father, Dr. Avol Fox Harris, a physician who confronts Caligari in a tense clash. The confrontation unleashes another shocking reversal: Dr. Avol is injected with Mrs. Van Houten’s brain fluid and undergoes a radical transformation, emerging as a transvestite nymphomaniac—an unsettling reminder that the mind-swapping procedure can alter not just traits but entire identities.

Sexuality sits at the core of the narrative, threading through the atmosphere and actions of the characters. The film portrays adult desire and erotic ambiguity with striking bluntness for its time, including topless scenes and moments of self-stimulation involving Mrs. Van Houten. Yet it stops short of graphic hardcore content, maintaining its R-rated boundary while delving into provocative themes and transgressive imagery.

In a climactic twist, Mrs. Van Houten orchestrates a dramatic reversal: she injects Dr. Caligari with her own nymphomaniac brain fluid and, using the fluid of Caligari’s ancestor, effectively supplants the doctor’s control with the patient’s. The result is a full-spectrum role reversal—doctor becomes patient, patient becomes doctor—and the asylum’s future is left in the hands of those who have become its inmates.
