Mia Baran Isabelle Adjani is a devout Catholic schoolteacher at a boys’ school near Pittsburgh, where her husband, Guy Baran Chazz Palminteri, holds the role of schoolmaster. A former nun, Mia also wrestles with cardiomyopathy, which adds a vulnerability to the already fragile balance of her life. Her colleague and confidante, Nicole Horner Sharon Stone, a fellow teacher at the same school, grows protective of Mia as the situation at home turns increasingly hostile. Guy’s cruelty extends beyond words; he targets Mia with both physical and emotional intimidation, leaving her squeezed between fear and a quiet resolve. As the mist of fear thickens, the two women decide to act, sensing that the only way to escape his control is to remove him from their lives for good.

With careful planning, the two women lure Guy to an apartment owned by a family friend of Nicole’s. The mood is tense, the room fills with tension and intent, and Mia administers a drug to soften his resistance. In a calculated move, they drown Guy in a bathtub and then shroud the body with a shower curtain before concealing it inside a wicker box. En route back to the school, Nicole’s luck falters when her car crashes in a pileup on the interstate, yet the wicker box containing Guy’s body goes unnoticed by arriving authorities. They drain the pool in a desperate attempt to locate him, only to discover that Guy’s body has vanished. The mystery deepens as they realize a potential blackmailer could be at work, an unseen force threatening to reveal their secret.

The tension escalates when a sheriff’s department bulletin announces the discovery of a John Doe in a nearby river. Mia goes to view the body at the sheriff’s station, but what she finds is not Guy. This moment draws the attention of Shirley Vogel, a retired police officer turned private investigator. Kathy Bates steps into the plot as a wary observer who is skeptical of the women’s story from the start. Shirley offers to investigate Guy’s disappearance, and her presence introduces a new dynamic—one that unsettles Mia and Nicole, who fear that someone is watching and possibly blackmailing them. The chill of paranoia grows when Mia discovers the shower curtain used to conceal Guy’s body hanging in her bathroom window, a chilling memento that seems to prove that someone else knows what happened.

Shirley confronts Mia with the accident report from Guy’s car and deduces that Guy was on his way to visit Mia in Pittsburgh on the day he disappeared. The fear is amplified when two videographers documenting an event at the school capture a moment in which Guy appears to stand in a building window, an image that seems to confirm the haunting possibility that he could return. Later, while the school’s basement is being explored, Shirley is assaulted and knocked out, an incident that further unsettles everyone involved.

That night, Mia discovers Guy floating in her bathtub, and she witnesses him rise from the water. Overwhelmed by terror, she collapses, seemingly experiencing a heart attack. Nicole arrives as the scene unfolds, and it’s revealed that she and Guy had orchestrated a plan to terrify Mia into a heart failure. Nicole confesses a murky regret, insisting that she had wanted to call the scheme off. As they stand over Mia’s body, Guy reveals that she is not dead, and he attacks both women, launching a brutal confrontation.

In the chaos, Mia manages to flee, only to be tackled by Guy at the poolside as he attempts to drown her. Nicole acts quickly, driving a garden rake into Guy’s head, sending him crashing into the pool. Nicole tries to revive Mia, but Guy pulls her into the water and strains to drown her as well. Mia jumps into the pool and, together with Nicole, drowns Guy, ending the immediate danger. They emerge from the pool, and Shirley arrives on the scene, prepared to protect her clients with a claim of self-defense. She punches Mia in the face, a harsh acknowledgment that truth and justice have tangled consequences in their desperate actions.
