Dharmadurai, Vijay Sethupathi, is known in his village as the lovable yet ever-mumbling drunkard, a man whose relatives—his brothers and brother-in-law who run a chit-fund business—often confine and mistreat him. Only his mother, Pandiyamma, and his steadfast friend Gopalaswamy stand by him, offering moments of care amid the constant trimming away of his dignity. One night, Pandiyamma overhears plans from Dharmadurai’s brothers to end his life, and in a quiet act of courage she helps him escape, urging him to start anew far from the village. He flees for Madurai, unknowingly carrying the chit-fund money that belongs to his family, a twist that will haunt him in the days to come. His brothers, upon realizing his absence and the missing funds, search in vain and eventually surrender to the reality that Dharmadurai is gone.

A flashback reveals the man’s path in the world of medicine. Dharmadurai becomes the village’s first college graduate, earning an MBBS at Madurai Medical College. There, he is part of a tight-knit circle with Subhashini and Stella, two women who harbor feelings for him. Inspired by the ethos of Dr. Kamaraj—the doctor who believes in serving the underprivileged—they make a quiet vow to remain in India after graduation, hoping to make a difference. Yet as life takes its course, the three drift apart, their friendship tested by time and circumstance. In the present, Dharmadurai seeks Stella for solace, only to learn she has passed away in a car accident. He then turns to Subhashini, now married to a Hyderabad-based doctor, who is shaken by the changes in him. He confesses the heartbreak and the descent into alcoholism that followed.

After graduation, Dharmadurai returns to his village and sets up a clinic to serve the needy. A chance encounter with Anbuselvi, a farm laborer who brings elderly women to his door, rekindles his longing for a simple, meaningful life. He asks her father, Paraman, for permission to marry, and Paraman agrees with hopeful optimism. Anbuselvi also agrees, and the couple begins to plan a future together. But Dharmadurai’s brothers demand a lavish dowry—gold and a substantial cash sum—which Paraman refuses. The wedding is called off, and Anbuselvi is driven to despair, choosing to end her own life. The blow pushes Dharmadurai into a deeper rage, a rage Pandiyamma tries to restrain. Heartbroken and betrayed, he abandons his clinic and falls back into the bottle.

Subhashini learns of Dharmadurai’s turmoil and reaches out, bringing him into her apartment where she helps him fight the grip of alcoholism. As they grow closer, she reveals her own wound from a husband’s betrayal and her decision to divorce; she confesses that she has long loved Dharmadurai since their college days, a confession he accepts with a quiet, mutual warmth. They decide to forge a life together, and Subhashini bravely ends her marriage so they can be together. Dharmadurai resumes his medical practice in the town, earning a reputation for his dedication to those in need. A breakthrough comes when a blind Dr. Kamaraj visits, inspired by Dharmadurai’s work. On Kamaraj’s suggestion, Dharmadurai and Subhashini decide to marry, and Subhashini becomes pregnant with their child. She encourages him to return to his roots, to reconcile with his family, and to share the news of their growing family with the villagers.

Returning to the village, Dharmadurai discovers that his family had to flee their home after it was sold to repay villagers who uncovered the embezzled funds. He finds Pandiyamma and his brothers living in a makeshift hut, broken by the years of hardship and the weight of their past actions. A moment of violence erupts when one brother strikes him with a spanner, but the moment shifts to remorse when the brother discovers that Dharmadurai has recovered the money. Pandiyamma rushes him to the hospital, where a former college classmate—someone who had clashed with him in the past—provides care and aid. As Dharmadurai recuperates, Subhashini calls to urge him to return home, and the couple waits eagerly for his arrival, ready to embrace the future they’ve chosen together.
