As the ravages of a deadly pandemic, sparked by a virus that reduces its victims to violent, zombie-like creatures, continue to decimate the United States (with 150,000 infected and 30,000 fatalities) and North Korea (where 8.5 million people are on the cusp of collapse), a lone figure stirs in a blood-soaked motel room in Seoul. The man, whose features are obscured by a cross-shaped scar on the back of his head, is abruptly interrupted by heavily armed CIA agents demanding to know the whereabouts of Dr. Jung Byung-ho (Jung Jae-young). The doctor and his daughter had been reported missing 19 days earlier while en route to North Korea, sparking widespread concern and intrigue.

As he struggles to recall even the most basic details about himself, let alone why he's in this precarious situation, an eerie female voice whispers cryptic instructions in his ear. The disembodied guide identifies herself as Han Jung-Hee (Jeong So-Ri), a trusted operative for North Korea's Labor Party, which has formed an unlikely alliance with South Korean forces to develop a treatment for the devastating virus.

With no memory of his past, our protagonist is forced to rely on the mysterious voice to navigate the treacherous landscape. She leads him out of the motel room and into a public bath, where he must confront hordes of infected individuals hell-bent on doing him harm. His resourcefulness and fighting skills prove crucial in surviving this initial onslaught.

As the van carrying NIS agents (North Korea's Strategic operatives) hurtles through the darkness, Han Jung-Hee reveals more about Dr. Jung Byung-ho's pivotal role in the fight against the virus. The doctor had successfully developed a cure for his own infected daughter, Ha-Na (Kim Bo-Min), by harnessing an antibody from her blood. This breakthrough discovery has been replicated on a large scale in North Korean labs, with plans to produce 100 million vaccine shots.

The disappearance of Dr. Jung and his daughter, accompanied by the CIA's alleged kidnapping of the pair, has sparked a dangerous game of cat and mouse between North and South Korea. As tensions escalate, it becomes clear that the fate of millions hangs precariously in the balance, as rival nations blame each other for the sinister events unfolding on the Korean peninsula.

As Carter navigates the treacherous landscape of his own identity, he's thrust into a mission to rescue Ha-Na from a North Korean facility where the production of a life-saving vaccine is underway. Initially perplexed by the circumstances surrounding his assignment, Carter is told that he's a naturalized citizen of North Korea, born in South Korea but having fallen deeply in love with a North Korean woman and subsequently marrying her. The revelation raises more questions than answers, particularly when he's informed that his memory center has been blocked at someone else's command.

Carter's journey takes a dramatic turn as CIA agents converge on the scene, insisting that he's actually Michael Bane, a former operative believed to have met his demise during a mission in Syria. Jung-Hee, a pivotal figure in Carter's life, seizes the opportunity to guide him towards freedom by allowing her signal to be tracked to his molar. As the clock ticks down to an impending explosion, she imparts a crucial piece of information: the molar will detonate in mere seconds, providing Carter with a narrow window of escape.

As he and Jung-Hee blend into the crowded streets, they watch the news unfold, revealing that Dr. Byung-ho has successfully reached the research facility in North Korea. Agent Agnes (Camilla Belle), a mysterious figure with her own agenda, approaches Carter and implores him to trust her - an instruction she subsequently reinforces by blocking Jung-Hee's signal to Carter. Tragedy strikes when Agnes is brutally shot down, leaving Ha-Na at the mercy of the CIA agents who have kidnapped her.

Enter Choi Yu-Jin, a heroic figure hell-bent on saving Ha-Na from the clutches of his former comrades. In a desperate bid to rescue the captive woman, he sacrifices himself in the process, but Carter arrives just in time to save Ha-Na's life. Meanwhile, thanks to Agnes' earpiece, Carter discovers that the CIA has conclusively identified him as Michael Bane and are now preparing to take his life.

Seizing the initiative, Carter systematically eliminates the entire CIA squad, his skills honed from years of experience. Jung-Hee then guides him towards an airstrip, where he boards a plane bound for North Korea, the stakes higher than ever before.

As chaos erupts on the plane, Director Kim's heroic efforts to safeguard Ha-Na and Carter are tragically cut short by the North Korean medic's treachery. The medic, driven by sinister motives, had been manipulating events from behind the scenes. Despite being mortally wounded, Director Kim takes down the medic in a valiant attempt to protect his passengers. In a stunning revelation, the medic confesses to working for the CIA, mirroring Carter's own clandestine affiliations. As Director Kim succumbs to his injuries, the medic detonates an explosive device, creating a gaping hole in the plane and sending it spiraling into chaos.

As panic sets in, the infected pilots turn on the main cabin, while the medic seizes the opportunity to make a daring escape with Ha-Na, hoping to leave Carter behind. However, the resourceful agent (Carter) anticipates this move and manages to grab Ha-Na and a parachute mid-air, executing a flawless rescue. On the ground, Carter is once again beset by CIA operatives, whom he must fight off with his trademark tenacity.

With Ha-Na safely in tow, Carter faces a new onslaught of undead hordes, which he dispatches with ruthless efficiency. As he regains his footing, he finds himself at the doorstep of North Korea, where he reunites with Han Jung-Hee and General Kim Jong-Hyeok (Lee Sung-Jae). Just as Carter is on the cusp of reclaiming his memories and fulfilling his mission, a shocking betrayal strikes. The North Korean military, led by General Kim, stages a coup, seizing control from Ha-Na's superior. With the virus in hand, General Kim plots to unleash devastation upon South Korea and the United States, intending to create a new world order under his own authoritarian thumb.

As General Kim's sinister plans unfold, he masterminds a coup while Ha-Na falls into military hands. Meanwhile, Carter makes a daring escape from Kim's soldiers and sets out to reunite with his estranged family at a bio-medical facility where Ha-Na is being taken. There, he discovers that Jung-Hee is none other than his long-lost wife, and in a tense confrontation, he shoots down General Kim who threatens her with a gun. This pivotal moment also marks the introduction of their infected daughter Yoon-Hee, whose erratic behavior and subsequent transmission of the disease to Jung-Hee sets off a chain reaction.

As the family fights for survival against both the infected patients and Kim's relentless soldiers, they make a desperate bid to escape the facility on a jeep. The quintet - comprising Carter, Jung-Hee, Yoon-Hee, Dr. Jung Byung-ho, and Ha-Na - embarks on a perilous journey to intercept a train operated by the Chinese government, which is transporting infected individuals to Dandong, China. With the fate of humanity hanging in the balance, Jung-Hee reveals that stopping the train is their only hope to prevent the virus from spreading globally.

As they navigate treacherous terrain and evade relentless pursuit, the Jung family manages to restore Carter's memories through a device implanted on his head. He recalls that he agreed to this mission in exchange for the freedom of his family - imprisoned by North Korea after discovering his true identity as an undercover CIA spy. Carter also remembers that he suggested blocking his own memories to prevent him from betraying North Korea.

Meanwhile, amidst a continuous cat-and-mouse game with General Kim's forces, Jung-Hee dispatches Carter, Ha-Na, and Yoon-Hee ahead to intercept the train, while she and Dr. Jung hold back the troops. Carter boards the train and engages in a fierce battle against North Korean operatives seeking to kill him and reclaim Ha-Na. Unbeknownst to Carter, General Kim has survived and is orchestrating a surprise attack.

Yoon-Hee awakens from her comatose state, seemingly cured of the disease. As Jung-Hee and Dr. Jung converge on the train, Carter kills General Kim in his helicopter before joining Jung-Hee, Dr. Jung, Ha-Na, and Yoon-Hee onboard. However, their reunion is short-lived as the train hurtles towards a catastrophic fate - the bridge ahead explodes, plunging the train into a downward spiral.
