Ray wakes up in a secret experimental facility operated by the corporation RST, led by the enigmatic Dr. Emil Harting. Harting reveals that Ray’s previous body was destroyed and now contains advanced microbots called Nanites—tiny machines capable of healing and regenerating injuries, even from severe wounds. This high-tech upgrade grants Ray superhuman strength and resilience, although he grapples with the shock of his new reality.

Throughout his rehabilitation, Ray interacts with other enhanced soldiers, including KT, who has her own bio-transplant, and Jimmy Dalton with prosthetic legs. As Ray begins to test his newfound abilities, he is haunted by memories, especially hearing “Psycho Killer” by The Talking Heads, which triggers a flashback to Axe killing him and Gina. Driven by these recollections, Ray harnesses his hacking skills to track down Axe, eventually leading to a fierce confrontation where Ray fights through gunshot wounds, displaying his surpassing strength and regenerative abilities. When Axe pleads for his life, Ray, resolute in his mission, executes him, solidifying his transformation into a relentless force.

The narrative takes a darker turn when Ray is subjected to manipulation by Harting and his team. They stage a series of simulations and replays of the hostage incident, with a fabricated villain, Nick Baris, played by Johannes Haukur Johannesson. As Ray is manipulated into doubting his memories, only KT opposes the deception, understanding that he is being exploited. The story reveals connections between Harting, Axe, and Baris, implicating a conspiracy that involves mind control and betrayal.

Determined to find the truth and reunite with what he believes is his wife, Ray infiltrates Baris’s compound, utilizing his hacking abilities and combat skills. During the assault, Wilfred Wigans, a technician played by Lamorne Morris, reveals he was coerced into working for Baris but secretly wants to help Ray. Wigans shares that he experiences the same fragmented memories, implying Harting’s manipulation. As Ray tracks down Gina, he learns she is alive but remarried with children, adding an emotional weight to his mission.

The climax arrives as Ray battles Harting’s forces, facing a series of brutal confrontations, including a showdown in a street where Harting fires a grenade at Ray. Miraculously, Ray regenerates from the explosion, though Wigans warns him that further hits may be fatal. In a final act of resolve, Ray catches Harting’s grenade, using it to destroy Harting and end his manipulative schemes. Wigans then revives Ray with a reprogrammed Nanite treatment, and the trio—Ray, KT, and Wigans—drive off into an uncertain future, united in their fight against unseen threats.
