Lou [Elena Kampouris], a newly recruited paramedic eager to prove herself on her first day. Her father, Patrick [Jeremy Piven], a retired police officer, supports her endeavors, while her superior, Rick [Sean Astin], remains skeptical of her capabilities. Lou is partnered with Jason [Jaime M. Callica], a seasoned paramedic, to navigate the chaos engulfing the city.

Amidst the chaos, a masked serial killer emerges, his silhouette and mask echoing Michelangelo’s David. The killer leaves behind victims in grotesque states, accompanied by cryptic Latin phrases scrawled in blood. Lou’s proficiency in Latin becomes instrumental in deciphering these messages, and she teams up with Detective Russo [Travis Nelson] to drive the investigation forward through the frenzied streets and burning sirens of the city.

As the body count climbs, Lou starts to sense a frightening personal connection to the killer. Her suspicions harden when she learns that the predator, David [Daniel Cudmore], has been stalking her and is tied to a traumatic event from her childhood. Years earlier, a fire deliberately set by corrupt officers, including her father, claimed the lives of David’s wife and son. David, long believed dead, survived with severe disfigurements and has since sculpted his vengeance into a singular, chilling motive.

David captures Lou, forcing her to confront the sins of her father. Patrick arrives in a desperate bid to rescue his daughter, but he is fatally struck by David. Lou escapes the suffocating grasp of danger and enters into a deadly cat-and-mouse pursuit with her relentless pursuer. Drawing on every resource at her disposal, she lures David into a trap, seizing control of a room and setting it ablaze. The flames roar around them, yet David proves stubborn, dragging the confrontation into a brutal final struggle where Lou’s resolve overrides fear, and she fatally stabs him, securing her own survival.

In the wake of the carnage, authorities place David’s charred body into a body bag, but a chilling detail lingers: his eyes remain open, as if even in death he refuses to surrender the story of what happened and who was truly responsible. The city begins to breathe again, though the scars of the past linger for Lou, a reminder that truth often arrives wrapped in smoke, confession, and the fragile line between justice and vengeance.
