A group of friends, consisting of Jason (Justin Chatwin), Neal (Danny Masterson), his assistant Theresa (Ashley Greene), Vick (Chris Geere), Joey (Alexis Knapp), Denise (Bar Paly), and Danny (Nick Thune), gather for a reunion on Eastman Island and make their way into Volcano, a nightclub buzzing with energy. Here, Jason has a fateful encounter with the enigmatic entertainer known as the Red Bastard. As he pursues him, he crosses paths with Xiomara ([Kea Ho]), a club dancer, and soon finds himself lured into a back room by the Man (Pierce Brosnan), who awaits him with sinister intentions.

The Man cryptically reveals that they have been anticipating Jason’s arrival, bursting into laughter that hints at more than mere amusement. What follows is an unsettling introduction to a new drug called Urge, which the Man describes as a key to unlock one’s hidden self, stripping away moral inhibitions. Jason’s impatience with the Man’s chatter leads him to brush off the warning, only for the Man to expose private knowledge about Jason’s life that leaves him shaken. After leaving Jason with the Red Bastard, the drug is presented with an ominous caution: it can only be used once in a lifetime.

Curious about his friends’ reactions, Jason shares the drug with them at a wild party that escalates quickly into chaotic debauchery. Stripped of their inhibitions, the friends quickly spiral into a nightmarish descent marked by violence and hedonism, with horrifying acts such as Neil abandoning Theresa after an encounter, carefree fights, public sex, and tragic deaths—including Denise’s demise at the hands of Vick, who is manipulated into a fatal confrontation. The indulgence of Urge spirals out of control, as it’s revealed that its influence has permeated the entire island, leading to madness and destruction.

As the harrowing events unfold, Jason manages to remain untouched by the drug and seeks refuge in a diner. By coincidence, he again meets the Man there, who coolly dismisses responsibility for the chaos around them.
