In the wake of a catastrophic chemical weapons attack that upends American life, the country divides into two stark factions and a new kind of social order emerges. The Gangs, a mosaic of rough factions with names like Gamblers, Sheets, Plowboys, Nailers, and Cherries, carve out their own territories, enforcing brutal control through fear and violence. Opposing them are the Domestics, a loosely affiliated group that longs for a return to peace and a life beyond constant threat and chaos. Interwoven with these strands is a voice of the outside world in the form of a talk show hosted by DJ Crazy Al, whose updates, commentary, and grim humor offer a running pulse on the state of the nation and the shifting balance of power.

The story centers on a married couple, Mark West and Nina West, whose divorce has been paused by the collapse of society and the perilous journey that follows. They head toward Nina’s parents’ home in Milwaukee, seeking safety while navigating a landscape strewn with dangers. Their trip is punctuated by moments of fragile respite, including a stop in an abandoned house where they are discovered by Nailers. A tense confrontation ends with Mark defending them by killing two intruders, a brutal reminder of the ruthless realities of this new world.

As they move on, Mark and Nina cross paths with Nathan Wood and his son Steven, who intervene during a harrowing chase by pursuing Nailers and invite the couple to their home for a communal meal. The visit brings them face to face with members of the Plowboys and, more chillingly, with rumors of a new gang composed entirely of children. The conversation takes a dark turn when Nathan reveals a horrifying truth: they have eaten human flesh. A nearby Nailer, surviving the earlier home invasion, tracks them, holds Nathan’s daughter hostage, and demands a trade for Mark and Nina. The confrontation erupts into chaos—Steven launches a counterattack, Nathan’s family suffers tragedy as his wife and daughter are killed, and Nathan, in a fierce moment of vengeance, kills the Nailer but vows revenge on the Wests. The tension escalates as Nathan rings his Plowboy contacts over a CB radio, and a heated exchange with Mark and Nina threatens to shatter their fragile alliance. A violent argument culminates in a gunshot that leaves Nina unconscious and Mark wounded.

Their next refuge is an abandoned house where they are followed by Betsy, a deserter Cherry with a fascination for Nina. Betsy’s past includes a brutal escape from Plowboy captivity, and she becomes a catalyst for a series of dangerous events, including Nina’s wounding. While Nina recovers, Mark searches for supplies in another dwelling, and Nina, waking up, strips away her wedding ring, contemplating a future beyond marriage. She discovers Mark’s notebook filled with pleas to give their relationship another chance, and a cache of records and an old Heavy Metal record sparks a moment of cathartic release as she dances to the music.

The danger intensifies when Mark encounters William Cunningham, a flamboyant but dangerous madman who has captured Nathan and Steven. William forces Mark to confront a brutal slave in a violent showdown; Mark’s quick, decisive action turns the tables, killing William and freeing the captive, and Nathan, in a surprising turn, chooses forgiveness and departs with Steven, leaving behind a path of possible reconciliation.

Reunited with Nina, the couple’s tenuous bond is tested again when Gamblers capture them and force a game of Russian Roulette at their clubhouse. Mark takes a shoulder wound, Nina seizes the opportunity to break free, and she shoots the host, allowing the two to escape in a stolen car. They eventually reach Nina’s parents’ house, only to learn from neighbors that her parents have died. Mark’s discovery of Nina’s father’s prized Wildey Hunter pistol missing adds another layer of loss and risk.

The danger moves closer as Nina learns that a neighbor named Phil is secretly aligned with the Gamblers. With this insider information, the Gamblers descend on the neighborhood, triggering a fierce gun battle. Betsy returns to support the couple with other neighbors and the Plowmen joining the fight in a bid to reclaim their community. In a brutal clash, the Plowmen and most of the Gamblers are killed. Betsy hands Mark the wedding ring Nina left behind at the abandoned house, only to be fatally shot by a remaining Gambler, a killer Nina then defeats with a precise shot of her own. Inside Phil’s house, Nina uncovers a corpse attributed to a stab wound in the abdomen; the killer is revealed to be another neighbor’s child, confirming the existence of the feared children’s gang. In the aftermath, Nina also finds her father’s Wildey pistol and the couple, battered but resolute, prepare to move on.

As they rest briefly at Nina’s parents’ home and then decide to keep moving, a radio broadcast by Crazy Al offers a closing update: a couple—marked by heroism and danger alike—have reportedly eliminated Gamblers in Milwaukee. The host frames their actions as a warning to other gangs and a glimmer of hope for the domestics, a sign that change, though perilous and costly, might still be possible in a divided nation.
