During the Second World War, the young RAF Flight Lieutenant Toby Jugg (Robert Pattinson) suffers a severe spinal wound during a bombing raid over Dresden, leaving him wheelchair-bound and facing a life as a paralyzed recluse. In the hope of recovery, his aunt-in-law, Julia Jugg (Rachael Stirling), takes him to Llancebach, a remote military hospital in Wales run by Dr. Hal Burns (Julian Sands), who is known for his unorthodox treatment methods.

Toby’s convalescence unfolds among shell-shocked veterans, and the atmosphere soon grows uneasy as other patients begin to die. Dr. Burns urges him to confront his own demons, but night after night Toby is plagued by horrific nightmares, reliving the carnage he helped unleash. Recurrent hallucinations and visions cloud his mind, and his only escapes are his cigarettes and the steady, caring attention of Sister Sally Grant (Melissa Lloyd).

Julia, with whom Toby had an affair, appears to be the only person he truly trusts. When an intern from the nursing home delivers a letter from Julia to Toby, he discovers that Dr. Burns has been hiding his correspondence with his aunt. At his request, Julia moves into the hospital to tend to her ward, but the doctor soon begins an intense, troubling affair with her.

One night, the doctor gives a drugged Toby a razor and tells him to do the right thing.

As Toby rejects further help from Burns, his grip on reality loosens further. He becomes increasingly suspicious and doubts everyone around him, weaving a dark, paranoid plan that involves Julia and Burns. In the depths of his madness, he ends up killing the one person who loves him, a tragedy born from fear, guilt, and a mind unraveling under the weight of war-trauma.
