Millicent, Morgan Saylor a college student who grew up in the foster care system, withdraws from school to finish her thesis and participate in a treatment program that uses microdoses of LSD. She accepts a job babysitting Johnny, Danilo Crovetti, a nonverbal young boy with severe allergies who must wear an astronaut-like suit to stay safe. In their new home, Millicent quickly learns that Johnny’s life is not as calm as it appears; his father Jacob is prone to philandering, while his mother Rebecca is determined to project the image of a perfect home.

Inside the house, Millicent steps into Rebecca’s role, becoming the caregiver Johnny begins calling “mommy,” a development that infuriates Rebecca. Millicent and Jacob begin an affair that Johnny witnesses, captured in his sketches of the encounter. Millicent also begins dosing Johnny with LSD, blurring the line between care and control as she imposes her own compulsions onto the child.

While Millicent journals her past abuses, the household dynamics grow darker: Rebecca discovers the affair through Johnny’s drawings and confronts Millicent, who attacks her and sets out to find Jacob. He refuses to leave Rebecca, leaving Millicent to contend with a fractured family that refuses to abandon its secrets. In Millicent’s absence, Johnny turns violent and follows Millicent’s prior instruction to harm his mother, attacking Rebecca.

Millicent returns home to find Rebecca and Johnny turning on her, and Johnny stabs Millicent to death. The film closes with a chilling revelation: this was not Johnny’s first murder, and his parents have long covered up prior deaths to protect their son—and they intend to continue to do so.
