In 1981, Dick Biel, a man with paranoid schizophrenia, escapes from a mental asylum after killing a doctor and stealing his uniform. Three years later, a sociology teacher at St. Trinian’s College is murdered while working alone in a classroom, and a new teacher, Julie Parker is hired by the school’s handicapped priest headmaster Dick Biel to take her place the following semester.

During her first weeks at St Trinian’s, Julie befriends fellow teachers Cynthia and Mark Hammond, and the two share a budding romantic tension as Julie settles into her role. Meanwhile, a chilling pattern emerges: two female students are killed by an unseen, knife-wielding killer—one in a local parking lot, the other at a drive-in movie. Cynthia phones Julie in the middle of the night to discuss the murders and her growing suspicions about Mark, and the two women grow closer as Julie decides to look into the mystery more deeply. With Cynthia’s help, Julie sneaks into Mark’s apartment while he’s out, uncovering a pocket knife and a stack of newspaper clippings about local murders. When Mark suddenly returns, Julie barely escapes his reach.

The next day, Cynthia’s mutilated corpse is found in a storage room within the college, and the building is closed early for the day. Julie, shaken and unsettled, tells Father Janson that she is quitting and begins to pack to leave town. Around the same time, more students and a teacher fall victim to the killer. Julie reaches out to Father Janson again, convinced that Mark is the culprit, and arranges to meet him at the campus later that night. When Mark arrives, Julie flees once more, racing through the halls as tension peaks.

As she confides in Father Janson about her misgivings, he reveals a shocking truth: he is not handicapped, but Daniel Grayham, and he has been the murderer all along—a revelation fueled in part by misogyny and religious zealotry. Drawing a knife from his crucifix, the killer presses his advantage, but Julie turns the weapon on him and escapes. Mark arrives on campus, having trailed Julie, but struggles to get inside. Julie hides in the bathroom, only to scream and rush out after stumbling upon the bloody remains of a teacher in one of the stalls. She then enters an elevator, but Janson catches up and kills Julie by slicing open her back.

Mark finally gains entry and discovers Julie’s body. He cleans away the traces, and Janson phones the police in a bid to frame Mark for the murder. However, Mark bursts in and notices a bloody knife sheath disguised as a crucifix on the wall of Janson’s office. With his plan foiled, Janson is taken away to an insane asylum, where he is placed in a straitjacket.
