Alex Monet, a Scanner, accidentally takes the life of his best friend while bravely demonstrating his abilities at a Christmas party. Devastated by the tragedy, he seeks peace and control in a Thai monastery, hoping time and discipline will help him harness the powers he cannot fully contain.

Two years pass, and Elton, Elton, Alex’s adoptive father, is pursuing a breakthrough: an experimental patch called EPH-3 (a variant of Ephemerol) designed to help Scanners suppress their powers and live more normal lives. His stepdaughter and Alex’s sister, Helena, who also bears Scanner gifts, longs to use the patch to quiet the migraines and the voices that plague her. Yet Elton cautions that the drug’s side effects must be thoroughly researched before any testing, even as Helena secretly slips on one of the patches in the dead of night. The moment the patch takes hold, Helena’s grip on reality begins to slip, and her already volatile mind is driven toward a dangerous edge.

The drug’s influence turns increasingly dangerous as Helena’s powers surge. She unleashes a brutal campaign of murders, beginning with Dr Baumann, a physician who previously subjected her to cruel experiments when she was a child. The trail of violence intensifies when she destroys Elton and seizes control of his pharmaceutical empire, using Eph-3 to assemble her own army of Scanners from Baumann’s patients.

Meanwhile, Michael, a trusted ally of Elton, travels to Thailand to locate Alex and warn him of the escalating threat. Tragically, Michael is slain by villagers under the mind control of one of Helena’s scanners, a reminder of how far Helena’s reach has extended. Alex receives a final training session from his monk mentor before returning to the United States to confront his sister and halt her rampage.

Helena discovers that she can bend her powers through television signals, a chilling expansion of her reach. In short order, she murders Mark Dragon, the owner of a TV network, and steps into his chair, using the platform to broadcast her abilities and extend her influence. As Alex lands back on American soil, he reconnects with his former partner, Joyce, and the two decide to join forces to stop Helena’s plan.

Together, Alex and Joyce confront Helena’s growing army of mind-controlled scanners and race to avert a mass broadcast catastrophe. Helena, intent on a global takeover, has planned to unleash the power of Eph-3 on live television, threatening to kill millions via the airwaves. Alex storms the TV station, rescues Joyce, and works to remove the dangerous patch from Helena’s body.

When the patch is finally removed, Helena’s mind reverts to something resembling her former self, and overwhelmed by what she has done, she commits suicide by electrically self-inflicting a fatal shock. In the aftermath, a final, unsettling twist lingers: Helena’s consciousness somehow survives, transferring itself into a video camera and lingering with a menacing smile as the screen fades to black.
