On the evening of April 30, in Layika Township, Hotan County, a simple family moment in a walnut orchard turns into a life-changing emergency. Xiaomai is playing near the fields while her mother and brother stay busy with farm work. A bamboo dragonfly, a small toy in her hand, slips away and slides under a running tractor. In a frantic bid to retrieve it, Xiaomai’s right arm is caught in the machinery and is tragically severed.

As night deepens, the family moves quickly to get help. Starting at 8:25 PM, Xiaomai’s mother and brother rush through rural roads and crowded bazaars to Hotan City, carrying the injured girl. They are aided by traffic police and emergency medical personnel who help stabilize the situation. By 10:00 PM, Xiaomai is on the operating table at Hotan Regional People’s Hospital, where doctors begin assessing the most urgent path forward.

In the delivery room, a critical decision is presented. Dr. Aikebai lays out two options: directly suturing the wound to save her life, or traveling to Urumqi for an arm replacement. The moment is overwhelming for the family. Xiaomai’s mother collapses in the hospital, overwhelmed by the weight of the choice. Meanwhile, Xiaomai’s brother weighs a different kind of risk—he chooses to pursue the arm replacement route in Urumqi, determined to give his sister the best chance at a usable limb.

The journey to save time becomes a coordinated national effort. The brother escorts Xiaomai toward the airport, but the clock is running. The last flight to Urumqi, CZ6820, is already preparing for departure. In a remarkable display of cooperation, Hotan Airport, China Southern Airlines, and Urumqi Airport—along with the flight crew and passengers on board—coordinate to bring the plane back to the jet bridge. With everyone’s cooperation, Xiaomai and her brother manage to board the flight just as the window closes, and CZ6820 begins its flight at 12:09 AM on May 1st.

The mission to shorten the journey requires additional terrain and airspace maneuvering. With help from the Civil Aviation Administration of China’s Xinjiang Air Traffic Management Bureau and the People’s Liberation Army Air Force, the plane flies a more direct route from Hotan to Urumqi, bypassing longer conventional paths. Inside the cabin, the journey remains tense: Xiaomai’s wound starts bleeding anew due to the pressure changes during takeoff and ascent, and she develops a pneumothorax after the tractor incident. The steady support of the crew becomes crucial. Flight attendant Zhou Yan and doctor Liu Rui provide continuous care, monitoring her condition and guiding the family through the ordeal.

As the aircraft closes in on its destination, every minute counts. The emergency center in Urumqi activates its plan to receive a patient in critical need. At 1:36 AM the plane lands in Urumqi, and just over half an hour later, at 2:12 AM, Xiaomai and her brother arrive at the Affiliated Hospital of Traditional Chinese Medicine of Xinjiang Medical University. The hospital is prepared for their arrival: preoperative exams are conducted, the amputated limb is cleaned and prepared, and blood is drawn in anticipation of the complex surgery ahead. Xiaomai’s operation begins at 3:09 AM, and after several hours of careful work, she returns to the ward at 7:13 AM. By then, her mother has also reached Urumqi, reuniting with Xiaomai and her brother after a night filled with urgency, hope, and resilience.

This remarkable medical journey—from a rural walnut orchard to a city hospital—highlights how coordinated care, swift decision-making, and dedicated professionals can turn a catastrophic accident into a fight for survival and a future possibility for recovery.
