The film follows Truman Gates, a former US Army Airborne Ranger raised in Appalachia, who relocates to Chicago to become a police officer. He’s married to Jessie, who is pregnant, and he’s forged a hard-won transition from a hillbilly past to a steady lawman present. The story is driven by the stubborn pull of family loyalties and a demand for justice that runs through both city streets and backwoods ancestral codes.

Briar heads to Chicago determined to find and avenge Gerald, while Truman tries to keep the peace and pursue justice within the system.
As Briar settles into a flop house, he hands the desk clerk his cousin’s number to call if he doesn’t return by morning, a small but telling sign of his precarious stake in this war. Briar’s hunt for Gerald’s killer leads him to shoot up a local mob hangout, signaling the raw violence that will push the two brothers toward a collision of ideologies. Truman arrives soon after; Joey—embarrassed by the assault—says he won’t press charges and stakes his own claim to “handle things” his way, a revelation that foreshadows the brutal path ahead.
A witness eventually helps the brothers piece together who hijacked the truck that killed Gerald. Truman pressures Lawrence, Papa John’s son, to turn state’s evidence against Joey. Lawrence seeks Joey’s help, and is betrayed, with his body later found tortured at the scene and Briar’s shotgun visible nearby. The mob’s response comes quickly: Papa John sanctions retaliation against the man they believe is responsible.
The violence escalates as Briar infiltrates Rosellini’s trucking company and engages Joey’s crew in a gunfight, killing two of them before Joey shoots Briar, leaving him fatally wounded and dying in Truman’s arms. A flop house desk clerk, who riffs on a life that’s heard too much, hears a news report about the deadly showdown and calls the number Briar had given him.
Even as the police and Truman see the signs that Briar’s prosecution was a setup and that his death happened in an ambush, there is no conclusive proof to convict anyone. Truman resigns from the police force to pursue the Rosellini mob directly, and the Gates family travels to Chicago to wage war against the Outfit. In a moment of grim, raw defiance, Truman hurls one of Joey’s men through a restaurant window, and Joey, abraded with resolve, discovers a chilling message on his car: “You forgot one.” He vows to kill Truman without Papa John’s permission.
The climactic confrontation unfolds in a dark cemetery, where a large-scale skirmish draws in both families. Truman pins Joey to the ground with a knife at his throat, but Papa John arrives with the Gates clan held at gunpoint and orders Truman to drop the weapon and step aside. Facing the truth about Lawrence’s murder, Papa John shoots Joey to end the blood feud and seal a fragile truce between the Gates and Isabella families.
Back at the police station, Truman finds Jessie and quietly affirms a new, chosen family in her and in the life they’ve built together:
