As time passes, Cream’s heart begins to open to someone outside their little circle—a wealthy dentist she says she loves. K bears a difficult mix of pride and sorrow at her news, yet he finds a fragile peace in the idea that she might be cared for by someone who can offer a long life and stability. His way of supporting her is to encourage a future where she is safe, even if that future means letting go of the intense, all-consuming closeness they share.

In a turn that tests both of them, K believes Cream would be better off with a life that doesn’t carry the weight of illness, and he asks Joo-hwan’s fiancée to end that relationship. The fiancée agrees, but only if K allows her to photograph him for a death-themed exhibit, a macabre exchange that underscores how fragile life has become for both of them. It is later revealed that Cream’s knowledge of K’s illness came not from direct confession but from her rummaging through his belongings to glean inspiration for a song—an act that unintentionally uncovers the deeper truth behind his quiet bravery. The items she discovers, she learns, are pain medications used for terminal cancer patients, a stark reminder of the fragility they are living with. Earlier in their story, Cream tests K by asking what defines a good man; his answer is simple and unwavering—a good man is one who can offer a healthy life to the person he loves. This sharpened understanding fuels a painful choice: Cream fakes falling in love with Joo-hwan in an effort to appease K, a deception she records and later confesses in her diary, a diary that becomes the lens through which their intertwined motives are laid bare.

As K’s health takes a sharper turn, the two spend Cream’s birthday together on what becomes their last day as a family. In a moment of tenderness and fear, Cream asks what K wishes for, and he replies with a line that is kept as a secret between them:

It’s a secret.

Cream teases that she doesn’t believe in such a promise and hints that the wish may already have been given years earlier. She holds onto the hope that, in the next lifetime, they might find each other again.

They share one final photograph, a poignant image of a family that has endured more than its share of losses. K passes away, leaving Cream to face the aftermath of a life shadowed by illness and sacrifice. In the end, Cream apologizes to Joo-hwan for the earlier deception, and Joo-hwan visits their grave, a quiet suggestion that Cream’s later suffering may have culminated in suicide after K’s death, a conclusion that underscores the film’s lingering questions about love, loyalty, and the limits of care.
