Max Matheson survives a devastating car crash that takes his fiancée Kayla, leaving him scarred by guilt and trauma. After working through therapy with Dr. Beaumont, he begins to find small steadiness in the wake of the tragedy, but his sense of safety remains fragile.

Max’s father, Jack, reopens the Mayflower Department Store in New Orleans and hires Max as a security guard, replacing the previous guard who quit after a crisis on duty. On his first day, Max meets the people who run and work at the store—Keller Landreaux, the store manager; Jenna McCarty, the buyer; and Ryan Parker, the vice-president of operations. The atmosphere is tense but professional, and Max tries to blend in as he copes with his memories and his uneasy new routine.

Before his first shift ends, Max experiences a startling vision: a dead woman appears in a mirror. Then he sees Jenna’s naked reflection in the glass, and it suddenly decapitates her. The fear he felt in the accident returns with a sharper edge as he realizes that the mirrors may be showing him real, impending deaths. The following night, Max glimpses Ryan’s reflection being disemboweled, and Jenna’s death becomes a cruel forewarning of what he might be able to foresee. He rushes to warn Ryan, but the tragedy unfolds just as he feared.

Haunted by the double shock, Max calls his father to tell him he’s quitting, but an even more frightening sight awaits him at home: Jack’s face is smeared with cuts and blood. A reflection almost kills him, and Max manages to intervene and save his father. This act cements a tense decision: Max will stay at the Mayflower, but he hints that he will do whatever the entity behind the mirrors demands if it spares Jack. The job becomes a test of his limits, and the store’s surveillance and the mystery behind the reflections begin to pull him deeper into danger.

When detectives Huston and Piccirilli question him about Jenna and Ryan, Max deflects, and they leave with more questions than answers. Yet the clues begin to point toward something supernatural. He returns to the main mirror, this time following a pale gleam of light that seems to guide him. The light leads him to a box containing the ID of Eleanor Reigns, a new hire who vanished two months earlier. A missing-person flyer confirms her disappearance, and Max reaches out to Eleanor’s older sister, Elizabeth, to uncover more details. Together they discover that someone deleted the surveillance files on the night Eleanor disappeared, and the store’s computer logs reveal a troubling gap in the record.

The pair track down Henry Schow, the former security guard who last accessed the file system. His mouth bears a scar that mirrors a mark left on his reflection, a clue that ties him to the mystery. Through Henry, Max and Elizabeth learn a devastating truth: Eleanor is dead. Months earlier, during the Mayflower’s groundbreaking party, Jenna drugged Eleanor while Ryan spoke with her, and Keller Landreaux raped Eleanor when she was intoxicated, killing her as she tried to flee. Henry found Keller burying Eleanor’s body in the basement crawlspace and was coerced into deleting the surveillance files to cover up the crime.

Eleanor’s spirit, trapped in the mirror world, haunts the living and seeks revenge on those responsible. She exploits Henry’s reflection to feed on shards of glass and to orchestrate killings—first Jenna, then Ryan. When she realizes Max can see her, she uses the threat to Jack to compel him to help contact her sister. Max accepts a fraught alliance with the ghost, understanding that his near-death experience has granted him the ability to glimpse the other side.

Max and Elizabeth return to the Mayflower to search for Eleanor’s body in the basement. Keller, realizing they know the truth, pursues them. He nearly strangles Elizabeth, but Max intervenes, recreating the near-death moment and allowing Elizabeth to witness Eleanor. After Eleanor appears again, Max wrestles Keller and hurls him into the main mirror. Eleanor acknowledges Max’s aid and pulls Keller into the mirror, condemning him to the fate he helped to obscure.

Back at the police station, Henry confesses his own role in the cover-up, and the impression is left that Keller has escaped capture and is blamed for Jenna and Ryan’s deaths. In a final, chilling moment, the interrogation room mirror begins to crack when Henry is left alone; he calmly sees Eleanor reflected there, and he accepts that his survival is over. Eleanor seals the scene with a final act of vengeance, killing Henry as the screen cuts to black.
