The film opens with a solemn blessing for the new building of Little Magnolia School. The mayor, among the guests, cautions Stanley Pardo, Dante Ponce, the school’s owner, about a dark memory from the past when a student died. Stanley’s wife, Faith Pardo, Iza Calzado, is inside the school’s restroom when a ghostly figure of a boy appears, hinting at the unsettled tragedy to come.

The next day, three grieving parents—Faith, Julio Sacdalan, and Stella De Castro, Jodi Sta. Maria—mourn the losses of their daughters: Maria Ann Pardo, Rhed Bustamante; Leonora Vera, Jonicka Cyleen Movido; and Teresa de Castro, Juvy Lyn Bison, following a field-trip accident. In an effort to help them cope, a psychiatrist named Manolo Apacible, Cris Villanueva, offers life-sized talking dolls to care for their children. At first, Faith and Stella refuse the dolls because they resemble their daughters too closely, and the voices disturb them; Julio, however, accepts the offer with a quiet resolve, trusting the perceived therapeutic effect.

As Julio begins to notice the doll’s impact, he shares his rapid progress with Faith and Stella. Faith, seeking solace, finally accepts the “Maria” doll, while Stella initially resists but relents when her husband Don, Joem Bascon, accepts a doll after Manolo presents it to him. The trio starts to feel a flicker of relief, a fragile comfort in their grief, but the mood darkens as hauntings creep into their homes and lives. Faith’s housemaid, Don’s housemaid, Don himself, and one of Julio’s students are all haunted in disturbing ways, and the school principal reprimands Julio for bringing the doll to class, which unsettles the children and their parents alike. Faith reveals that she is pregnant, a revelation that unsettles the already-tense balance of the group, and Stanley suggests discarding Faith’s doll to “solve” the issue.

Yet the dolls grow more dangerous, turning from therapeutic to lethal. Leonora kills the school principal, and Maria plants pencils along a staircase to lure Faith into a dangerous trap. The plan is foiled when a housemaid intercepts Faith and defuses the setup. Teresa escalates her violence, targeting Don and his mistress, yet a rapid series of screams pulls authorities into the scene. Don is gravely wounded and ultimately dies, while Maria targets the vigilant housemaid who foiled the scheme and stabs her, even capturing the moment before collapsing. Teresa murders her grandmother by driving a sword from a St. Michael statue into her forehead, a brutal culmination of the dolls’ deadly influence.

As fear and accusation swirl, Faith and Stella blame Julio for unleashing the dolls’ presence, though Julio denies involvement. Faith recollects the school’s dark history and the trio’s complicity in the events that followed; Julio storms off, insisting he did not cause the tragedy. Faith’s other housemaid then brings in Augusto, a witch doctor, Joey Paras, who conducts a séance to uncover the ghost’s true identity. The séance reveals Eldon Jacinto, the student who died years earlier, as the haunting force.

Manolo’s motives come into sharper focus: Eldon’s ghost is tied to Manolo’s past, and the boy’s death becomes a catalyst for vengeance. Eldon possessed the uncanny ability to glimpse the future, sharing a vision of the school incident with his classmates. When Eldon was silenced and erased, the truth was buried by money and lies; Stanley and Faith helped cover up the tragedy, silencing Eldon’s guardian and allowing Manolo to pursue his own revenge from abroad.

Armed with this knowledge, Julio and the others confront Manolo, only to be overpowered by the dolls. The trio fights back, destroying their own dolls, while Augusto squares off with Manolo. In a brutal confrontation, Manolo overpowers Augusto and impales Julio with a spear. Faith and Stella retrieve Eldon’s body for a proper burial and blessing, and just as all seems lost, Julio reappears to land a fatal blow, killing Manolo before succumbing to his own injuries. The final moment shifts to a bittersweet resolution: Faith gives birth, and Stella adopts Don’s illegitimate son, offering a new start for the two families.

In the mid-credits scene, another grieving parent faces the same pain, and Julio appears once more, extending a chilling parallel by presenting her with a Leonora doll, suggesting the cycle of loss may begin anew.
