Mafia! unfolds with a deliberately nonlinear, flashback-driven narrative that weaves together Tony’s present-day peril with the long arc of his family’s rise and fall. The film opens with Tony, the son of a powerful Mafia don, Vincenzo Armani Windbreaker Cortino, stepping away from a Las Vegas casino and delivering a chilling voiceover about his philosophy of life. As he starts the car, it explodes in a brutal, sudden burst that sets the tone for a story about shattered loyalties and fractured dreams.
The tale then rewinds more than fifty years to reveal Vincenzo’s boyhood in Italy, where he is the awkward son of a Sicilian postman. A chance delivery goes wrong when a parcel hides a mysterious white powder, and the delivery boy’s fate becomes the catalyst for a life-changing turn. The recipient, convinced the boy knows too much, pursues Vincenzo to a street fair, where Vincenzo’s world changes forever as the killer destroys Vincenzo’s father. The film then shifts to more recent events: Tony has just returned from the Korean War and accompanies his idealistic Protestant girlfriend, Diane, to meet his family at his big brother Joey’s wedding reception, a playful parody of a moment from The Godfather. The celebration is interrupted by a brutal reality as Vincenzo survives a nearly fatal gun attack—47 bullets intended to end his life. Tony vows to strike back against Gorgoni, a drug lord Vincenzo had refused to work with, while Diane confronts Tony with the cost of his violent world and his allegiance to his family. After avenging the assault, Tony hides in Las Vegas, where Cesar Marzoni invites him to manage his casino, The Peppermill. The arrangement brings Tony professional success but also introduces a dangerous distraction in Pepper Gianini, a femme fatale planted by Marzoni to erode Tony’s focus and widen the rift between Tony and Joey.
