Following the events after the prequel, Satya, Sudeep runs an orphanage for the downtrodden and, at the same time, continues to siphon off black money. The juxtaposition of his charitable work and criminal covert acts creates a tense, human portrait of a man pulled between mercy and motive, duty and deception. The film follows how his world is tested when a young resident named Jaanu enters his life and changes everything.

Jaanu, a nine-year-old girl in the orphanage, suffers from Brugada Syndrome, a dangerous heart condition. In a bid to save her, Satya takes her to Warsaw for treatment, where a fragile alliance begins to form. There, she befriends Dr. Priya, Madonna Sebastian, and with Jaanu’s hopeful energy guiding them, a bond builds among the trio. The medical urgency is clear, but so is the growing sense that Jaanu’s courage might be the key to unlocking Satya’s locked past—and perhaps softening the walls around his hardened heart.

Meanwhile, in Warsaw, Devendra, a ruthless pharmaceutical mogul, Nawab Shah hatches a chilling plan: spread a dangerous virus and offer a cure at an exorbitant price to reap enormous profits. Satya consults with the doctors about Jaanu’s operation, weighing the risks against the stakes of Devendra’s looming profit scheme. The clock ticks as Satya arranges the operation date and, on a brief detour from medical concerns, the trio visits the Aviation Museum before returning to India to continue their work in the shadows of a world that prizes wealth over humanity.

A new threat arrives in the form of a masked thief known as Ghost, who carries out a daring heist of the Crown Jewel—an item bought by Devendra and set for delivery within a week. A tense citywide chase unfolds as the police pursue Ghost, who miraculously escapes in the chaos. The pursuit deepens the mystery: the thief bears a striking resemblance to Satya, and he recites a Sanskrit mantra as the thefts and explosions unfold, suggesting a plan beyond mere robbery.

Interrogations begin as Sharath, Aftab Shivdasani, and Kangana, Shraddha Das, two Interpol officers, pore over CCTV footage and photos. They discover the masked doppelgänger of Satya, and the investigation pushes them to Bangalore, where they uncover the past of ex-ACP Kishore, P. Ravi Shankar. Kishore’s revelations pull back the curtain on Satya’s history, hinting at a web of loyalties and betrayals that stretch beyond the present crisis.

Devendra intensifies the chase by sending his assassin Michael after Satya to capture the Crown Jewel, but Satya outmaneuvers him and kills Michael in a brutal confrontation. The manhunt culminates in an arrest: Satya and Kishore are detained by Sharath and Kangana for questioning, with Priya’s world suddenly tilting toward heartbreak as she learns of Satya’s alleged scams and takes Jaanu with her to Poland for safety and treatment.

Poland becomes a new battleground. Ghost’s influence—along with Satya and Kishore’s fates—leans toward a desperate escape. The escape sequence brings them to a hospital where Jaanu is admitted, and Satya fights to prove his innocence. Anantha Krishna, a surgeon, attempts to sedate Satya, but the resourceful man breaks free and convinces Priya and Kishore that he is not the villain of the tale. The truth begins to surface: Anantha Krishna’s role in Ghost’s criminal network is more complex than anyone anticipated, and the mysterious Ghost is revealed to be Shiva, Satya’s twin brother, a revelation that reshapes the entire narrative.

Through a series of flashbacks, the history of Durga, the HDO officer and mother to Satya and Shiva, comes into sharper focus. Durga opposed Devendra’s virus-and-cure plan, but Devendra murdered her. The brothers’ origins are laid bare: Satya is born healthy, while Shiva bears nerve damage that marks him for a different fate. Their father Prakash returns from abroad to oversee the brothers’ future, and Anantha Krishna provides for Satya while taking Shiva to Poland for study. Shiva’s resolve to avenge their mother grows stronger as he learns of Satya’s path and resolves to bring him to Poland to reveal the truth.

In the present, Jeet, Devendra’s son, abducts Jaanu from the hospital. Satya tracks Jeet via a tracker planted by Shiva and rescues Jaanu, though the mission ends with an ambulance explosion that destroys the vehicle’s oxygen supply and kills Jeet. In the ensuing chaos, Shiva/Ghost and Kishore are captured by Interpol, only to be kidnapped by Devendra’s henchmen. Shiva reveals himself to Devendra and eliminates him and his associates, attempting to frame Kishore for the crimes, who is arrested as a consequence. Satya is released and returns home with Jaanu, bearing the scars of the long pursuit.

In the final reckoning, the Crown Jewel is sold, and the proceeds are redirected toward healing those harmed by Devendra’s medicine. Shiva uses the newfound wealth to fund cures and medicines, committing to a future where power and profit no longer come at the expense of human lives. The story closes on a note of tentative reconciliation and a renewed vow to protect the vulnerable—the orphanage’s wards, Jaanu’s health, and the wider community that Satya and his allies endeavor to shield from exploitation. Chameli, Ashika Ranganath, makes a fleeting cameo that punctuates the film’s broader world, a reminder of the interconnectedness of every life touched by these events.
