Rupert Grimm learns that there are several students ahead of him in line to be admitted to Johns Hopkins School of Medicine, and he hatches a plan to arrange a string of unfortunate “accidents” for them, a scheme that casts a cold, calculating shadow over the campus.

Gabriel Higgs also seeks entry, driven by a family legacy that has long reserved the medical profession for his bloodline. He tries to bribe both Amanda Morel and Randall Burns, but they refuse and respond with a cool arrogance that only fuels his resolve. If it wasn’t for the hacking skills of his ally Ron, Gabriel might have fallen off the list entirely.

Gabriel then turns toward Kirby Watts, hoping to win her favor; instead he falls in love, though she rebuffs him, leaving his pursuit unfulfilled.

The tragedy unfolds: Randall meets a grisly end when he tries to rinse off after getting chemicals on his clothes; the shower instead releases a caustic liquid that engulfs him in flames. Amanda dies after consuming a morel fungus to which she is allergic, a pair of deaths that feel linked to their surnames.

With the body count rising, Gabriel becomes the prime suspect, though he manages to persuade Kirby that he is not the killer. Rupert lures Kirby into a dangerous trap and outlines his grand plan to murder everyone on the list while framing Gabriel.

In the climactic showdown, Gabriel acts to save Kirby and unmask Rupert, bringing the killer to justice. After the dust settles, the survivors begin to forge a new future: Randall and Amanda are gone, Kirby is admitted to the program, and she marries Gabriel to carry on the family name. Gabriel takes up a role as an ecology professor, and Rupert’s corpse is donated to Kirby’s class for scientific study.
