Two years after Dracula II: Ascension, Father Uffizi (Jason Scott Lee) and Luke (Jason London) discover that Dracula has returned with Elizabeth Blaine (Diane Neal) to his castle in the Carpathian Mountains. But the road ahead is blocked by doubt: Cardinal Siqueros (Roy Scheider) refuses to grant his blessing, suspecting Uffizi may be tainted by the vampire’s influence. Faced with this resistance, Uffizi renounces his priestly vows and sets out with Luke for Bucharest, determined to confront the ancient evil on his own terms.

Romania lies in the grip of civil war, its streets lined with NATO peacekeepers and the country fractured by conflict. In an abandoned village, Uffizi and Luke stumble upon a crashed helicopter that carried a news team—Julia Hughes (Alexandra Wescourt) and her cameraman Tommy. Tommy is turned by the nightmare of vampire clowns that plague the area, but the two vampires are swiftly dealt with by Uffizi and Luke. They must decide what to do with Julia, whom they leave behind, only to be drawn into a rebel trap that tests their loyalties and their resolve. The rebels, hungry for a story rather than a rescue, refuse to release Julia to England without a tale about vampires in a country devastated by war.

As night falls, the undead strike the rebel base with terrifying force. Uffizi, Luke, and Julia manage to survive the assault and press onward toward Dracula’s castle, where Elizabeth Blaine lingers on the edge of Dracula’s dark world, nearly fully converted to his way of life. Inside the fortress, Dracula mortally wounds Julia, and he taunts Uffizi with the claim that forgiveness alone can grant true death. With resolve, Uffizi confronts the ancient monster in a climactic duel: he first drains Dracula of his blood, then beheads him, declaring that he has earned forgiveness for the life he took. In the castle, Elizabeth’s fate is sealed by Luke, who beheads her at her own request. Luke departs the fortress, while Uffizi ascends to the throne, with Julia revived as a vampire by Dracula’s fall. The closing moment leaves a chilling implication: Uffizi may have succeeded Dracula as the new lord of the undead, with Julia by his side as queen.
