Dominick Luciano (Tom Hulce), known as “Nicky,” and Eugene Luciano (Ray Liotta), known as “Gino,” are fraternal twins whose lives are defined by family duty and a fragile memory. Nicky has a mental disability, and Gino, a medical student, carries the weight of caring for him while navigating a tempting offer to study at Stanford University Medical School. Gino fears that if he leaves, Nicky may struggle to cope on his own, creating a delicate balance between ambition and devotion.

Nicky works as a trash collector for Mr. Johnson, played by Bill Cobbs. His routine is steady and dependable, yet it hides the emotional strain of watching his brother’s world tilt toward crisis.
