The minions of ChromeSkull locate their employer and bring him back from the brink of death, though the injuries have left him disfigured. As he recovers, he is tended to by his loyal assistant, Spann. At the same time, his disgruntled second‑in‑command, Preston, hunts down the survivors from the previous film—Princess and Tommy—trailing them to a motel. A deadly confrontation follows: Princess is killed, and Tommy is spared because he’s out at the time of the attack.
Three months pass, and Preston has grown increasingly disillusioned with ChromeSkull’s leadership. Spann works to maneuver himself into a better position by brownnosing the boss and helps orchestrate a new victim for ChromeSkull: a legally blind waitress named Jess. The plan includes setting up a hideout inside an electroplating plant. In a chilling turn, ChromeSkull breaks into Jess’s house, kills her visiting friend, and captures Jess herself.
The police catch a glimpse of ChromeSkull on a video Jess shot, prompting them to bring Tommy in for a re‑interview. That move ends up saving Tommy, as Preston had slipped into his apartment disguised as ChromeSkull and murdered Tommy’s roommate.
Jess wakes to a nightmare—the coffin-like confinement of a compartment in the factory—while Preston taunts her and orders two workers to forge a new weapon: a spring‑loaded knife with six blades. Following a lead, a detective is sent to the factory and becomes the test subject for Preston’s novel weapon. Preston abducts Tommy, leaves Jess bound, and learns that ChromeSkull has fired him for copying the leadership style and methods.
The two killers clash in a brutal showdown, and ChromeSkull emerges victorious by decapitating Preston. ChromeSkull seizes Preston’s weapon, incapacitates Jess and Tommy, and presses his attack on an investigating detective along with a squad of officers. The FBI arrives and takes over the case as ChromeSkull makes his escape from the factory. The scene then shifts to Hollywood, where he is seen speaking with Spann, who assures him that they will locate Jess.
In the post‑credits sequence, the FBI confronts ChromeSkull’s pregnant wife, who is unaware of her husband’s crimes. Once the truth comes out, the terrified wife seizes an agent’s gun and shoots herself in the mouth.
