As an annual ritual, Bob and his circle of friends engage in the "Buddy Games", a series of challenges designed to foster camaraderie and strengthen bonds. The reigning champion, Shelly, has triumphed six times, including a recent paintball showdown against Bender that left him victorious. In the aftermath of their raucous celebrations, Shelly's boorish behavior prompts Bender and Nikki (Shelly's wife) to jest about exacting revenge by targeting his most vulnerable area. However, it is later revealed that Bender had indeed aimed the gun at Shelly, but Nikki intervened, ultimately leading to an unforeseen consequence: Shelly loses both testicles, a secret he keeps hidden.

Five years pass, and Shelly's life has become a desolate landscape. His marriage to Nikki has imploded, leaving him to pick up the pieces of his shattered existence. His mother, desperate to save her son from self-destruction, reaches out to Bob, now a wealthy entrepreneur, with a plea: revive the Buddy Games to give Shelly a reason to keep going. When Bob broaches the idea with Shelly, he agrees on one condition: Bob must not reveal his secret physical impairment and Bender must be excluded from participating.

Bob extends an invitation to their friends (excluding Bender), who respond with unbridled enthusiasm. However, his girlfriend is less than thrilled about Bob's decision to participate in the Buddy Games once more, leading to a heated argument that culminates in her departure via helicopter. Meanwhile, Bender discovers the return of the Buddy Games through whispers among their social circle and surprises Bob at his opulent mansion. To spare Bender's feelings for being excluded, Bob fabricates a tale about having to pay a $10,000 entry fee to participate this year. He also drops a bombshell: Shelly has suffered a catastrophic loss, one that will forever alter the dynamics of their friendship.

As the group ventures deeper into the secluded cabin, they're suddenly interrupted by Bender's uninvited arrival. He reveals his desperation, having sold his late mother's possessions and rendered questionable services to the homeless in exchange for entry. His admission is met with a somber tone as he also discloses that Shelly had undergone a traumatic ordeal involving testicular loss. The revelation sparks an emotional outpouring from Bender, prompting Bob to confess the truth – he hadn't actually invited Bender, instead opting to spare his friend's feelings. With hurt and anger simmering beneath the surface, Bender goads Bob into offering a substantial cash prize for the winner of the competition, highlighting each friend's pressing financial needs, including Shelly's own desire for parenthood. Shelly begrudgingly extends an olive branch, allowing Bender to participate in the games.

The inaugural day of competitions unfolds with a series of challenges, commencing with an eating contest, followed by mini motorcycle racing, a thrilling water slide experience, a muddy obstacle course, and a chaotic watermelon smash. Bob emerges victorious from the first leg, courtesy of Shelly's missed finishing line flag following the water slide. The next challenge presents each friend with a daunting task: consuming an entire bottle of laxatives before attempting to pick up a woman at a bar. Bender outmaneuvers his friends, employing cunning tactics to secure victory. As the games progress, the friends participate in a surreal competition, where they smoke a joint and affix a raw steak to their heads. The room is then infiltrated by a komodo dragon, prompting a battle of wits and endurance as each friend strives to outlast the reptile's attacks. Shelly ultimately emerges victorious, his mental state seemingly compromised as he bites the dragon's tongue.

The trio of Bob, Shelly, and Bender advances to the final stage of the games. The night preceding the grand finale, they celebrate with pina coladas, oblivious to Shelly's underhanded move – he has secretly spiked the drinks with Bender's sperm. As the group watches a commercial featuring Durfy, their mocking laughter only serves to further inflame his frustration. Seething with anger, Durfy ventures into the woods, where he is brutally attacked by a rabid animal. Bob, Doc, and Zane subsequently embark on a search for Durfy, ultimately finding him and convincing him to perform an impromptu scene, effectively reviving his flagging acting career.

As the drama unfolds at the cabin, Bender's slumber is abruptly disrupted when Shelly seizes the opportunity to exact revenge. With a cunning grin, she binds him to a tree with duct tape, rendering him helpless. The next morning, Shelly takes her vendetta to new heights by deliberately crashing Bender's beloved van - a poignant reminder of his late mother's thoughtfulness and his own struggles since her passing. But Bender's world is turned upside down when he reveals the truth: Nikki was the one who pulled the trigger.

The following day, the competition reaches its climax as the three finalists don protective gear and engage in a high-stakes game of cat and mouse in an open field. Bob's supportive girlfriend, Tiffany (Tiffany), arrives on the scene, but her efforts are quickly derailed when Bob is eliminated from the competition with a direct hit to the hand. As the last two contestants face off, Shelly gains the upper hand, only for Bender to demonstrate his remarkable reflexes by snatching the arrow mid-flight. The battle between them culminates in a brutal hand-to-hand combat, which ultimately results in Bender emerging victorious and claiming the cash prize.

Tiffany's subsequent proposal to Bob is met with a resounding rejection, sparking a fiery outburst that culminates in her taking down Bob, Doc, Zane, and Durfy. As the dust settles, Tiffany's pent-up emotions are finally unleashed, leaving her companions reeling.

In the end credits, a heartwarming moment unfolds as Zane confesses his true nature to Doc and Durfy secures a role on a popular CW show. Meanwhile, Shelly continues to distance herself from Bender, only for him to kidnap her and reveal that he has used part of his winnings to procure artificial scrotums - a bizarre yet fitting gesture in their twisted game of one-upmanship.
