The film opens in a hospital, where a man named Siva is rushed into the operating theater after a devastating fire left his face badly deformed. The doctors believe a face transplant could restore his appearance and give him a new start. At the same time, another patient named Shakthi is admitted for a different fate: he is declared brain dead, and his mother Gowri Thiruselvan sits with the doctors as they request her consent to donate his organs. When Gowri agrees, Shakthi’s face is transplanted onto Siva, and with his new look, Siva awakens to a second chance at life and a renewed sense of responsibility toward his family. He hopes to use this chance to build a business and provide for those who rely on him, but danger quickly shadows his recovery.

Almost immediately, Siva is targeted. He is attacked twice by masked strangers, and a third assault forces him to confront a disturbing truth: someone seems to be chasing him not for who he is, but for the face he now bears. The risk becomes more personal when Gowri reveals a painful secret: Shakthi, the donor, was not her son. This revelation prompts Siva to search for answers about Shakthi’s past, and he learns that Shakthi’s origins lie in a village near Trichy. To uncover the truth, Siva enlists the help of his friend Gopal and his girlfriend Gigi, who had once rejected Siva before his transplant due to his altered appearance. During a train journey to Trichy, a candid moment unfolds when Siva speaks openly about his new identity, and Gigi, moved by the truth of his sacrifice, asks for forgiveness, sparking a reconciliation between them.

In Trichy, the trio digs into Shakthi’s life while another attempt is made on Siva’s life. They chase the culprits through the city, and a fateful accident occurs when Siva accidentally hits a pedestrian while fleeing. Taking responsibility, he admits the person to a hospital, where they meet Maya, who becomes their guide to Shakthi’s world. Maya explains how Shakthi, Shanmugam, and Maya once worked for a multinational company, only to face layoffs and a turn toward village life and farming. Shakthi believes farming can be a path to dignity, but the trio’s plan grows into a broader project: to interlink rivers by building a canal that would redirect water from a flood-prone village to their own drought-stricken area.

Their ambitious project faces stiff resistance from local authorities, police, and a scheming councilor named Mayilvaganam, who sees land and power slipping away from him. The three friends rally the villagers to support the canal project, arguing that leveraging surplus water from one river could irrigate the nearby fields and restore farming livelihoods. After initially pushing back, the district collector is swayed by logic and public support, and formal permissions begin to materialize. But tragedy strikes when a landslide—triggered by a hidden device planted by the antagonist—buries people and claims Shanmugam’s life, a devastating blow that also fills the group with guilt over the unintended consequences of their movement.

Back in the present, Maya divulges that she never heard from Shakthi after that episode, and she helps connect the dots about the river project’s hidden dangers. Siva, inspired by Shakthi’s dedication, delves into Shakthi’s laptop, unlocking it with his own face and discovering a trove of contacts who stood with Shakthi and the villages—a list that includes an unsent email to Maya and a video in which Shakthi speaks in his final moments. The video reveals the grim truth: a traitorous Sub-Inspector has colluded with Sooraj, a powerful man intent on privatizing the region’s electricity supply. Sooraj manipulates the official channels and uses his influence to derail Shakthi and his friends, employing hired goons to sow violence and suppress dissent.

A climactic confrontation unfolds as Siva pursues Sooraj to safeguard the truth. The Sub-Inspector is killed in the ensuing struggle, and Shakthi’s allies endure grave injuries as Sooraj’s schemes threaten to topple the fragile balance between progress and community welfare. In a tense sequence aboard a train, Gigi hints at a deadly drug that can induce an overwhelming adrenaline rush and fatal suffocation, leaving no trace in the blood. Siva later uses this knowledge to confront Sooraj, killing him in a carefully planned act of vengeance that mirrors Shakthi’s last wishes: to honor the lives lost and ensure the villages’ voices are not silenced.

In the film’s closing moments, Siva chooses to continue Shakthi’s unfinished work. He steps off a bus and heads toward Shakthi’s village, carrying with him the legacy of the man whose face now carries both a new identity and a heavier responsibility. The story ends with a quiet resolve: a life saved and a life committed to a cause beyond personal survival, as Siva embarks on a journey to build a better future for the communities that Shakthi fought to protect.
