Basket Case 3: The Progeny 1991

Duane Bradley, [Kevin Van Hentenryck] is removed from Belial after his crude attempt to sew the monstrous former conjoined twin back onto his side, and he ends up confined in Granny Ruth’s haven for the “unique”—a padded cell, a straitjacket, and a sense that danger still lingers in the shadows of their odd little world. Months pass, and Granny Ruth decides it’s time for a road trip to the home of her ex-husband, Doctor Hal Rockwell, so that he can assist Eve, Belial’s grotesquely shaped girlfriend, as she prepares to give birth to their brood. The plan comes with a stern warning to Duane: stay far away from Belial, who has started to shut Duane out telepathically after the failed reunion attempt.

On the way to Hal’s house in Peachtree County, the group makes a stop at a drug store where fate nudges several key players into motion. Sheriff Griffin meets Granny Ruth and is momentarily swayed by her charm, while Duane, desperate to escape, tries to wriggle out of a bus window and recruit Opal, Opal the sheriff’s daughter, to help them flee. But Granny Ruth’s quick return to the bus and her persuasive talk to Sheriff Griffin ensure they keep to the planned route, even with a bus parked illegally in a small-town street.

Arriving at Hal’s house, tension thickens as Duane finds a window to freedom and slips away, only to be recaptured as the day unfolds. Eve’s delivery moment—the birth of Belial’s many offspring—becomes a grotesque, charged event, with Hal in surgical attire triggering a flood of memories about the original separation. The birth is overseen by Little Hal, Little Hal the multiarmed, blob-like prodigy son of Hal and Granny Ruth, and the scene blends fear with a strange, almost reverent awe. Belial is briefly drugged to keep him calm, a necessary pause before the brutal consequence of new life and new danger.

Meanwhile, Deputies Bailey and Baxter, Bailey and Baxter, catch wind of a million-dollar reward tied to the capture of both Duane and Belial and break into the Rockwell residence. They stumble into a grim tableau, mistake Eve for Belial in the dim rooms, and shoot her, a fatal error that deepens the already fragile grip of the freaks on their world. The revelation of Eve’s death sets a somber mood at the police station, where Sheriff Griffin must contend with the tragedy and the mounting chaos.

The station becomes a battlefield of personalities as Belial launches a savage attack and Duane, released from confinement by Opal in a bid to stop the rampage, finds himself once again at odds with his violent twin. Officer Brennan, Officer Brennan, is overwhelmed as Belial crushes his throat and the scene turns grisly when Banner loses his head to the creature and Brody narrowly escapes a similar fate when Belial corners him. Desperate measures lead to a pivotal turn: Duane aligns with Belial, and Little Hal crafts a robotic exoskeleton that lets Belial brutally retaliate against the police, mowing down Bailey and Baxter in quick, brutal fashion.

As Eve lies dead, Granny Ruth steels the freaks for what comes next and they march toward closure in a controversial, thunderous fashion. Eve’s funeral becomes a catalyst for a larger plan: Granny Ruth negotiates with Sheriff Griffin, offering Belial in exchange for the babies. The exchange is set at an abandoned factory on the town’s edge, and the atmosphere crackles with the tension of a final, high-stakes confrontation. The staff at a nearby fast-food restaurant scurries in fear as the group raids the place, a symbolic purge of the town’s normalcy in favor of the stories that lurk in the shadows.

Belial and the group converge at the factory where the deal is to be sealed, and a dramatic clash ensues. The exoskeleton grants Belial a terrifying edge, and the sheriff’s confrontation culminates in a tense, heeled moment when a recovered Sheriff Griffin fights to prevent the monster from reclaiming its newborn progeny. The life-and-death struggle ends with the babies exposed to danger in their cradle, but the freaks’ determination won’t let fear win the day.

With the babies finally reclaimed, Granny Ruth, Duane, Belial, and the rest of the freaks descend on a TV talk show in a final, defiant strike against oppression. Belial savages the host as Granny Ruth steps into the glare of the camera and delivers a bold, uncompromising message to the world: the freaks will no longer hide in the shadows, and they will fight back if they are pushed too far. The film closes on Granny Ruth’s resolute command to the audience to have a nice day, a darkly ironic wink at the world that has just watched a story of fear, family, and revenge unfold in haunting, memorable fashion.
