Dr. Ravi Tharakan [Prithviraj Sukumaran] is a renowned cardiac surgeon working in a private hospital. The story opens with him facing a trauma-filled case: a young girl is brought in, and Ravi volunteers to operate for free despite the wishes of her father. The operation ends in tragedy, the girl dies, and her father along with a chorus of local politicians erupt in protest outside the hospital. Ravi flees through the back door, but a subsequent accident leaves him missing without a trace. As the death attracts media attention, the police intensify their search, and the hospital management attempts to shield itself by terminating Ravi. Yet the chairman’s secretary Diya [Rima Kallingal] expresses deep dissatisfaction with the decision and launches her own inquiry, drawn to Ravi and reminded of his old conviction that ethics must guide medicine.
Diya’s investigation leads her to Dr. Vivek [Narain], a very close friend and former classmate of Ravi. Together they dig into a past where Ravi and Vivek were among the worst students at their private medical college—unable to distinguish instruments and often floundering in their early days. Ravi’s first romance at college was with Sainu [Samvrutha Sunil], a junior Muslim student, also known as Sainaba. After graduation, their options were stark: pay the college ₹5 lakh or accept an internship in a village hospital. Vivek chooses to pay, while Ravi, lacking funds from his father Thomas, heads to Redemption Hospital in Munnar, a place run by Dr. Samuel [Prathap Pothan], a physician whose unwavering devotion to his vocation makes his life feel almost solitary. On the way, Ravi’s car collides with a police vehicle, and he becomes a target of harassment by ASI Purushothaman [Kalabhavan Mani], whose grievance leads to Ravi’s suspension after a complaint is filed.
Ravi initially finds Redemption Hospital difficult—Samuel is strict, and the environment feels almost prison-like. The only glimmers of solace come from letters from Sainu and occasional calls to Vivek. He is soon joined by Dr. Supriya [Remya Nambeesan], a junior doctor who helps him rediscover the purpose of patient care, but the weight of Samuel’s stern approach remains. Months pass, and Samuel reveals he received a call from Sainu’s parents planning to marry her off. Samuel urges Ravi not to meet her, and Vivek secretly arranges Ravi’s and Sainu’s marriage in Kochi, only to be stopped by Purushothaman during a routine check. Ravi arrives late to learn that Sainu is house-arrested and has reluctantly agreed to another marriage, a blow that hits him hard and reshapes his outlook.

Samuel testifies on Ravi’s behalf, asserting that the girl’s life was saved and that there was no negligence in the end. This testimony becomes a turning point for Ravi, sparking a profound change in his outlook on life and the responsibilities of a healer. He leaves Redemption Hospital to pursue higher studies in London, gradually building a reputation as a service-minded physician who places others before himself.
