Sharyn Berkley, Ashlynn Yennie, arrives at a hospital with acute appendicitis and faces emergency surgery, only to awaken to a world that feels strangely unfamiliar. She is soon introduced to Dr. Aaron Hellenbach, [Louis Mandylor], whose calm explanation about her healing progress clashes with the visible reality of her nearly fully healed incisions. The explanation hints at unconventional healing methods, and before she can process it all, she attempts to flee her room, only to be sedated by the facility’s staff.

Inside the facility, Sharyn encounters a group of fellow patients — Cassandra Franklin, [Augie Duke], Rizzo, [Christos Vasilopoulos], Marjorie, Lester, [Alexi Stavrou], and Everett, a criminal attorney who claims his tongue was cut out but grew back. Cassandra shares the blunt truth of their situation: they’re in an experimental program, and Sharyn herself was burned alive. Rizzo is chosen for a procedure during the mysterious “Indexing,” and he reveals that he has already endured a brutal impalement in the past. The atmosphere grows increasingly eerie as the line between medicine and something far more sinister blurs.

Hellenbach discovers Sharyn wandering the halls and escorts her to his office, offering a telephone call if she talks about her ex-boyfriend, Nico, a drug trafficker who is said to have died by suicide. When the call fails to work, Sharyn explores the facility and stumbles upon a piano room containing a noose. As punishment for breaking the rules, her leg is amputated and reattached, a stark reminder of the control the facility exerts. Rizzo articulates a chilling theory: the military is harvesting test subjects to develop immortal soldiers. Desperate to escape, Sharyn makes another futile attempt to flee.

Everett tries to reassure Sharyn that cooperation will lead to release in three months, but the situation spirals the moment she admits she saw Nico alive in the building after his supposed death. Everett confronts Hellenbach, who vows that Sharyn will be evaluated in three months; when Everett pushes back, his tongue is severed again in a grim display of power.

The two women, Sharyn and Cassandra, hatch a plan to steal Hellenbach’s keys and study a map of the facility, hoping to navigate their way to safety. Their stealth is short-lived, as staff quickly corners them in the corridor. Cassandra tries to end her torment with a self-inflicted slit throat, creating a distraction that lets Sharyn break away. Rizzo steps in to shield her from a violent Lester, but the same sneaky corridors reveal that Rizzo has already committed a grave abuse of power: he molests Sharyn in a hidden room during the Indexing, and she fights back by tearing away his testicles.

Sharyn’s escape triggers an alarm, and she makes a dash toward a residential wing, only to be pursued by a staff member who returns her to the institution. Upon waking, she finds herself hanged in the piano room, and she asks why Hellenbach keeps healing her instead of killing her—the answer is chilling: the facility does not kill people.

Cassandra eventually confides a harrowing confession about her own torture and immolations: during life she broke into a mink farm, released the animals, and set the farm ablaze, killing occupants who remained inside. She argues that death may be the only escape, but Sharyn counters that they might already be dead, trapped in Hell. The next layer of truth emerges as Sharyn admits she killed Nico ten years earlier and framed it as suicide, a revelation that reframes the entire nightmare.

Confronting Hellenbach, Sharyn learns that everyone in the facility is technically a patient — including him. He reveals a long history of overdoses caused by overprescribing opiates to his patients, and explains that he arrived in Hell in 1969, where he overdosed on bile and now lives a cycle of punishment. After three months, each patient is allowed to begin tormenting others and the cycle continues until they comply. The discussion leaves Sharyn wondering if there might be a permanent way to die, but Hellenbach insists there isn’t.

Into this maze steps Nico, who reappears to pull Sharyn from Hellenbach’s grasp. They hurry through the halls, only to be confronted by the doctor. Nico overpowers Hellenbach, and the pair reach the edge of a cliff above a pool of lava. Sharyn bids Nico farewell and steps toward the edge, hoping for a way out. A miraculous path materializes across the lava as doctors revive Sharyn back on Earth. She reaches the other side, but Nico cannot follow.

Back in the hospital, Sharyn discovers that she survived only because she was placed in a medically induced coma after the surgery and was clinically dead for roughly fifteen seconds. Her husband and daughter enter the room, offering a fragile sense of normalcy.

Once home, she uncovers articles online that corroborate the strange tale: notes about Dr. Hellenbach confirm his existence and methods, and entries about Cassandra Franklin point to her death from mushroom poisoning. A final moment of quiet reflection comes when Sharyn visits Nico’s grave, paying respects to the man she could not save and who saved her in a nightmare she will never forget. The truth remains a haunting blend of medical science, moral ambiguity, and the eerie possibility that Hell, or something like it, may be closer than anyone wants to admit.
