Annavaram is a hard-working blacksmith living in a remote village, where his beloved sister Varam dreams of a decent life after marriage. He confides in his friend Narasimha that he wishes for his sister to thrive in a good city once she is wed. When a city suitor proposes to Varam, Annavaram agrees to the match and travels with the newlyweds to Hyderabad, where fate gifts him a surprising turn: he falls for Aishwarya after a string of comic and chaotic mishaps.

In the bustling city, Annavaram’s courage is tested when he saves a court witness from Tappas Balu, a feared don who holds sway over central Hyderabad. He also uncovers Puranapool Ganga, a local don who dominates northern Hyderabad and disrupts Varam’s husband’s canteen business. A dramatic clash at Golconda Fort leaves Narasimha dead at the hands of Ganga’s elder brother, a brutal revelation that shakes everyone involved. After Narasimha’s funeral, Annavaram leaves his hometown under the pretense of securing a job at a cloth manufacturing company, but in truth he sets off on a personal mission to cleanse the city.

Adopting the alias “Pothuraju,” Annavaram begins a ruthless campaign, wiping out 20 of Ganga’s henchmen and a corrupt sub-inspector who serves the don. He warns Ganga that he will be his next target and vows to wipe out all the dons in Hyderabad, criticizing the police for their inaction. The revelation comes to a police inspector, Sairam, who is challenged by Annavaram to capture at least one thug to stop him. The plan becomes deeply personal when Sairam’s son is killed by Balu, driving the inspector to cooperate more closely with the crusade.

With renewed resolve, Sairam helps map the mafia network across the city, listing the leaders of each territory and the chain of command. Annavaram records these names and designates Balu as his first target, setting in motion a deadly game of cat and mouse. Balu receives a chilling letter from Pothuraju threatening his life within 24 hours and seeks protection from the authorities, but ACP J. D. Yadav refuses, criticizing Balu’s prior harassment of the public. In a grim turn, Balu abducts Yadav’s daughter, forcing Yadav to confront the threat. Disguised as a wounded man, Annavaram is captured by Balu’s henchmen and, in a bold reveal, identifies himself before Balu. In a decisive confrontation, he kills Balu and his men, proclaiming that he has begun the “slaying of demons” and freeing Yadav’s daughter in the process.

Aishwarya encounters Annavaram again at a temple and discovers he isn’t employed at a cloth factory after all. He fabricates a cover story, claiming to work at a travel agency, and she agrees to visit him whenever possible, cementing a fragile bond between them. The crusade intensifies as Annavaram targets Gutkha Pandu, challenging him to face the consequences of his reign. Pandu hides in a politician’s residence, and the politician actually damages his own property to lure police protection. Seizing the moment, Annavaram infiltrates the politician’s house and, with the aid of a crowd rallying against a fictitious financier, kills Pandu.

As the net tightens, ACP Yadav questions the rally participants, but they refuse to reveal Pothuraju’s true identity, viewing him as a hidden savior rather than a villain. Yadav’s daughter, who was saved by Pothuraju, also declines to reveal who he is. With the mafia’s grip weakening, Annavaram announces a final plan to kill Ganga on December 31. He sends Ganga a coffin and funeral posters, prompting a furious reaction from the don who seeks protection from a massive network of thugs. In a bold strategic move, Annavaram disguises himself as a police inspector and orchestrates a fierce clash at Ganga’s fortress, with Sairam’s help. A ruse involving a dead thug provokes a violent police assault, masking Annavaram’s true actions.

As the New Year nears, Annavaram strikes at Ganga, stabbing him and hurling him from a window before the police can grasp what happened. The city rings in the new year in celebration of the apparent triumph, unaware of Annavaram’s exact involvement. In the end, the brave crusade finds its emotional closure as Aishwarya welcomes Annavaram, along with Varam and her husband, into a hopeful future after his mission is complete.
