During the turbulent times of World War II, Kurt Gerstein, a dedicated officer in the Waffen-SS and employed at the SS Hygiene Institute, finds himself caught in a moral nightmare. His primary role involves designing methods to purify water and eliminate vermin, but everything changes when he discovers the horrifying truth behind the use of Zyklon B, a hydrogen cyanide mixture he helped develop. Instead of being used solely for disinfection, the same chemical later becomes a tool for mass murder in extermination camps, aiming to eradicate Jews and other groups deemed undesirable by the Nazi regime.

As Gerstein grapples with this shocking revelation, he desperately attempts to expose the atrocities. His first effort is to notify Pope Pius XII, hoping that the Vatican might take a stand. However, he is met with a disappointing silence from the Catholic hierarchy, which issues only a weak and vague condemnation of Hitler and Nazi Germany. Only one person within the church, Riccardo Fontana, a young Jesuit priest, shows a genuine concern for the truth and the moral urgency of the situation. Together, Gerstein and Fontana try to raise awareness about the mass extermination of Jews, but their efforts are met with limited response and significant hesitation from those in power.

Gerstein’s determination leads him to travel to Rome to speak directly with the Pope, but he is ultimately denied the opportunity. Meanwhile, the situation in Italy worsens as German forces begin to seize control of Rome and deport Italian Jews to the death camps. Fontana pleads with the Pope to intervene, even suggesting he accompany the Jews to the train station to challenge the deportations physically. Yet, the Pope refuses, citing fears of causing hardship for Catholics living under Nazi rule. In a powerful act of defiance and compassion, Fontana puts on a Jewish Star of David badge and voluntarily boards a train heading for the camps.

Upon arriving at the extermination site, Fontana is detained by the camp’s commandant, known only as the Doctor, who is an influential and seemingly sympathetic figure within the Nazi hierarchy. Despite knowing Fontana is a Catholic priest, the Doctor permits him to stay among the prisoners and work in the crematorium, but ultimately, he is ordered to be gassed. Gerstein, attempting to save his friend, pleads with the Doctor to let Fontana go, but the request is denied. The Doctor then escorts Gerstein out of the camp, leaving Fontana and the remaining victims to face the mass murder.

The film depicts harrowing scenes as Gerstein and the other prisoners witness the brutal disposal of bodies—mass graves being dug up and set on fire by German soldiers. The Doctor later asks Gerstein if he has contacts to help him escape Germany, revealing his own fears about the advancing Allied forces. Returning home, Gerstein compiles all his evidence documenting the Nazi atrocities, risking everything to deliver the truth to the Allies. Unfortunately, his efforts are not enough to save him. Despite the solid proof, Gerstein is arrested, and after being formally charged, he is found dead in his cell—hanged at the hands of the regime he once served.

After the tragic end of Gerstein, the Doctor is shown seeking assistance to flee Germany. In a conversation with a Cardinal in Rome, he claims, “I’m a doctor, just a physician,” and receives help to escape to Argentina, highlighting the complicity and moral ambiguities faced by many during that dark period of history.
