In a tense hospital dawn, a man named Shiva faces a grim reality: his face has been grievously damaged after a fire, and doctors propose a drastic remedy—a face transplant from a brain-dead donor. The donor is Shakthi, whose life is cut short, and the medical team pleads with Gowri Thiruselvan to consent to the transfer of Shakthi’s face to Shiva. The operation marks a turning point for Shiva, who awakens with a renewed lease on life and a clear plan to support his family by starting a business. Yet behind the relief lies a dangerous shadow: someone seems intent on ending Shiva’s life, launching a series of brutal attacks that leave him wary and increasingly paranoid.

As the threats escalate, Shiva begins to suspect that his attackers may be fixated on his new face rather than his identity alone. A third assault confirms his growing suspicion, and it becomes clear that the fixation on the transplanted visage may be more dangerous than any threat to Shiva himself. With the help of his friend Gopal and his girlfriend Gigi, Shiva digs into the mystery behind the transplant and discovers a troubling truth: Shakthi’s donor may not have been Gowri’s actual son. This revelation spurs Shiva to seek answers far from home, leading him to the city of Trichy.

On the train to Trichy, the estranged relationship with Gigi surfaces again; she had previously rejected Shiva based on his appearance before the operation, and their reconciliation on the journey marks a fragile but hopeful turning point in their partnership. In Trichy, Shiva and Gopal press for information about Shakthi’s life, while a new threat emerges with another attempt on Shiva’s life. A chance encounter brings them to Maya, who recounts Shakthi’s past and explains why people are after Shiva.

The revelation unfolds in layers. Shakthi, along with his friends Shanmugam and Maya, once worked for a multinational company but found themselves cast aside after internal conflicts left them unemployed. Shakthi’s decision to return to farming brings him into conflict with local powers, particularly the scheming councilor Mayilvaganam, who tries to force farmers to sell their land. The trio rally the villagers around an ambitious river interlinking project: they aim to connect a flowing river 20 kilometers away to help their own drought-stricken village by channeling surplus water through a canal. Despite resistance from officials and police, the villagers unite behind them, and a public demonstration at the district collector’s office eventually yields permission to begin construction.

As construction starts, a sudden landslide devastates the project area, burying many people and claiming Shanmugam’s life among others. Maya’s later confession reveals that she had not heard of Shakthi after that day, and the memory of those events weighs heavily on her. Returning to the present, Shiva pores over Shakthi’s laptop, unlocked with his new face. There, he finds details about the supporters of the canal project, including residents of the neighboring village, and an unsent email to Maya. In a final, haunting video, Shakthi explains the truth: a bomb had been used to trigger the landslide, planted by a man named Shanmugam before his death, and the chain of corrupt interests reaches high into local power structures.

The story threads together with the discovery that the Sub-Inspector who helped them is a traitor and that Sooraj is at the center of the conspiracy. Sooraj has manipulated Shakthi’s world to privatize electricity in the region and to punish those who would stand in the way. In a brutal confrontation, the Sub-Inspector is killed while Shakthi fights to stay alive and sends a video message to Maya; Shiva witnesses the devastation and vows revenge. In a climactic turn, Sooraj tries to close the net by offering a counterfeit path to a peaceful life, but Shiva has a different plan. He learns about a drug that induces an extreme adrenaline rush, causing suffocating death without leaving traces in the blood, and uses it to kill Sooraj, avenging Shakthi’s life.

The film closes on a note of quiet resolve. Shiva, driven by the memory of Shakthi and the people he touched, steps away from a life of danger and toward Shakthi’s village, where he intends to continue the unfinished work of the man whose face he bears. The final image suggests a future built on resilience and renewal, with Shiva choosing to live for others and to carry forward Shakthi’s legacy.
